Two combos that include parvo and distemper (your puppy had the first from our vet at 6 weeks with a booster at 8 weeks, the next one is due at 12 weeks and the final at 16 weeks. Then wait 2 weeks before giving the rabies vaccine.

No, your puppy is fully dewormed

Yes, it is in between their should blades on their back, but can move around. It will be registered in your name with Heather Thompson (breeder) as an alternate contact as part of the contract you will sign.
