By signing below, I confirm that all information provided in this application is true, accurate, and complete to the best of my knowledge. I understand that if I am selected as a Coach or Instructor at Volusia Sports Center, any false statements or material misrepresentations may result in immediate termination of my association with the facility.

As a condition of serving as a subcontractor, I authorize Volusia Sports Center to conduct a background check, which may include, but is not limited to, reviews of sex offender registries, child abuse records, and criminal history databases. This screening is conducted in accordance with Volusia Sports Center’s child protection policies. I acknowledge that my selection is contingent upon the successful completion of this background check and understand that any concerning findings may disqualify me from consideration.

I acknowledge and agree that, if selected, I will be required to review and sign Volusia Sports Center’s Molestation Policy and Concussion Awareness Policy as a condition of being present in the facility or on the property in any capacity.

I hereby release and agree to hold harmless Volusia Sports Center Corp, Volusia Sports Center and Batting Cages LLC, and their affiliated partners, associations, officers, employees, and volunteers, as well as any organization or individual providing background check information, from any liability arising out of or related to this process.

I further acknowledge that Volusia Sports Center is under no obligation to hire or contract with me, regardless of previous employment or experience. If selected, I understand that I am expected to comply with all Volusia Sports Center policies, procedures, and standards of conduct, and that any violation may result in suspension or termination of my role.

Additionally, I confirm that the contact information provided in this application is accurate and up to date. I grant Volusia Sports Center permission to use this information for necessary administrative, operational, and promotional communications during my tenure as a Coach or Instructor.
Instructor Commitment, Setup Fee & Compensation Acknowledgement

At Volusia Sports Center, we invest significant time and resources to set our instructors up for success. If selected as an instructor, you will be required to pay a one-time $50 initial setup fee to help cover the costs associated with establishing your profile and marketing your services.

This setup includes:
- Setting up your online booking in our Square system
- Adding your instructor profile and booking links to our website
- Creating your professional bio and personalized marketing flyer (printed copies will be provided to you for distribution like a business card)
- Featuring you on Volusia Sports Center social media platforms, with potential paid advertisements
- Announcing your role through email marketing campaigns
- Providing one Volusia Sports Center instructor t-shirt (a $20 value) to get you started
*Additional shirts may be purchased at a 50% employee/staff discount

This setup process requires a minimum of 3-5 hours of staff time to complete. The $50 setup fee will be deducted from your first paycheck. If your first paycheck does not fully cover the fee, the remaining balance will be deducted from your next paycheck.

If you are not committed to actively marketing yourself and securing lessons, please do not submit this application. Instructors are expected to take an active role in growing their client base—this is a partnership, not a passive opportunity.

By submitting this application, you also acknowledge and agree to the following:
- You will be paid on a bi-weekly basis (once every two weeks)
- You are classified as a 1099 independent contractor, not an employee
- You are responsible for your own taxes, including but not limited to federal, state, and self-employment taxes
