By signing below, I confirm that all information provided in this application is accurate and complete to the best of my knowledge. I understand that if I am selected as a Coach or Instructor at the Volusia Sports Center, any false statements or material misrepresentations may result in the immediate termination of my association with the facility.

As a condition of serving as a subcontractor, I authorize the Volusia Sports Center to conduct a background check, which may include but is not limited to reviewing sex offender registries, child abuse records, and criminal history databases. This screening is conducted in compliance with the Volusia Sports Center’s child protection policy. I acknowledge that my position is contingent upon a satisfactory background check, and I understand that any concerning findings may disqualify me from consideration.

I hereby release and agree to hold harmless the Volusia Sports Center Corp, and Volusia Sports Center and Batting Cages LLC, along with their affiliated partners, associations, officers, employees, and volunteers, as well as any organization or individual providing background check information, from any liability related to this process.

I further acknowledge that the Volusia Sports Center is under no obligation to hire me, regardless of previous employment or experience. If selected, I understand that I am expected to adhere to all Volusia Sports Center policies and principles, and any violations may result in suspension or termination of my role.

Additionally, I confirm that the contact information provided in this application is accurate and up to date. I grant the Volusia Sports Center and its partners permission to use this information for necessary communications and promotional purposes during my tenure as a Coach or Instructor.
Instructor Commitment & Initial Setup Fee

At Volusia Sports Center, we invest time and resources to set up our instructors for success. If selected as an instructor, you will be required to pay an initial setup fee to cover the costs of establishing your profile and marketing your services.

This includes:
- Setting up your online booking in our Square system
- Adding your profile and booking links to our website
- Creating your bio and personalized marketing flyer (printed copies provided for distribution)
- Featuring you on our social media accounts with possible paid advertisements
- Announcing your role via email marketing campaigns
This process takes a minimum of three hours for our staff to complete. To ensure applicants are serious about conducting lessons and utilizing these resources, a setup fee will be deducted from your first direct deposit or paycheck. This fee may be deducted in stages or as a one-time charge.

If you are not committed to actively marketing yourself and securing lessons, please do not submit this application. We expect our instructors to take an active role in growing their client base—this is a partnership, not a passive opportunity.
