Complete the form below to RSVP for the June 16, 2022, July 15, 2022 and August 12, 2022 Assessment Days.
Please note: There is a $20.00 evaluation fee.

We appreciate that swimmers are eager for their results at the conclusion of the assessment. Results will be emailed approximately 1 week following the assessment day.

June 16 assessments will take place at White Oak Pool.

July 15 assessments will take place at Queen Elizabeth Pool.

August 12 assessments with take place at White Oaks Pool.

Upon completion of this form please be sure to arrive at the pool at least 10 minutes prior to the start of the assessment that you selected above. Swimmer must have a swim suit, cap and goggles.
