I have read the Bright From the Start Licensing Manual and understand the rules and regulation that govern Dragon Academy.
I have read the Employee Manual and understand the policies and procedures that are required of me as an employee of Dragon Academy.
I have received three Dragon Academy Shirts. If I am not with the company at least 1 year I will owe Dragon Academy $22 per shirt that will be taken from my final paycheck.
If Dragon Academy has paid for CPR/First Aid or other training and I am not with the company for 1 year, I will reimburse Dragon Academy for the cost of said training.
If an employee leaves Dragon Academy without providing a two week notice, the employee's final paycheck will be reduced to minimum wage for any remaining payrolls.
I have been shown the Workman's Compensation list of doctors that is hanging in the breakroom of Dragon Academy.
