If you are a family with older children already in the program, but you are also registering a new Level 1 student, please provide a copy of that student's baptismal record. PLEASE UPDATE CLASS ACCOMMODATIONS/MODIFICATIONS INFORMATION: If any of the students you are registering have special needs, please indicate below. If yes, please explain what the primary diagnosis is so we are better able to serve you. For example, please indicate if your child's primary diagnosis is Autism, A.D.D., A.D.H.D, Reading Disabled, Cognitive Speech Delay, O.D.D., Sensory Processing Disorder, Cystic Fibrosis, etc. This information will be kept confidential.

OLS Faith Formation Program Late Returning Family Tuition Information for registrations received between June 1, 2025- September 1, 2025:
- One Child: $110
- Two Children: $160
- Three or More Children: $175
After September 2, 2025, a $50 late fee will be added to the tuition for Returning Families for the current program year. Registration opened June 1, 2025 and our onsite class option will close on October 1, 2025 for the 2025-2026 Faith Formation Program Year. Our Home Study option will remain open. Onsite class spots are limited after September 2.

Sacrament Offerings are now built-in to registration payments-
- Level 2 child (First Communion Year offering): please add $50
- Level 9 child (Confirmation Year Offering): please add $75
