I release Café Salsera, its employees and agents from any and all liability for failing to hire me or terminating my employment due to false information or material omissions made by me. I authorize the companies, schools, or persons named above to give to Café Salsera any information regarding my employment or educational background, together with any information they may have regarding my qualificatins for the job which I am applying, whether or not it is in their records.     I hereby agree, upon a request made under the drug/alcohol testing policy of Café Salsera, to submit to a pre-screening drug or alcohol test and to furnish a sample of my urine for analysis. I understand and agree that if I refuse to submit to a drug or alcohol test under company policy, or if I otherwise fail to cooperate with testing procedures, I will subject to immediate termination.

    I UNDERSTAND AND AGREE THAT IF I AM HIRED, MY EMPLOYMENT RELATIONSHIP WITH CAFÉ SALSERA IS AT WILL, WHICH MEANS THAT IT MAY BE TERMINATED AT ANYTIME, WITH OR WITHOUT CAUSE OR ADVANCE NOTICE, BY EITHER ME OR CAFÉ SALSERA. In addition, if I am hired, Café Salsera will have the right to impose discipline or alter my position at its discretion. I understand and agree that no representative of Café Salsera may enter into any agreement contrary to the foregoing unless it is done by way of specific writtenagreement signed by the owner/president of Entity Café Salsera.
