I understand that it is required by tribal policy, all offers of employment by the Eastern Shoshone Tribe must first submit to a controlled substance testing which involves collections of a urine sample and will be tested for the following controlled substances: marijuana, cocaine, opiates, amphetamine and phencyclidine (PCP).

I further understand, if I test negative for use of controlled substance, I will be hired. I understand that, if the test is positive for the use of controlled substance. I will not be hired.

The result of the drug test will be maintained by the medical review officer on contract with the tribe, who will report to the Tribe whether the test results were negative or positive. The result of any test will not be released to any additional parties, except as provided by Tribal or Federal Law, without my written authorization.

I hereby agree to have my child submit to a pre-employment drug test.
