This registration form can be utilized for registering multiple participants for a single Yoga 4 Classrooms workshop or training with or without using a Purchase Order. If may be helpful to read through this order form before completing it so that you are familiar with the items needed to register your group.

If you are trying to register for a workshop or training sponsored by UNH PD&T or other hosting organization, please return to open the relevant training product page and choose the appropriate registration link. We can not accept registrations for our trainings and dates hosted by other organizations.

An Excel spreadsheet or Google sheet roster must be provided to include the following information/columns for each participant: First and Last Name, Role/TItle, Grade Level/s, Email Address. It is very important that name spelling, email address and other information included in the roster is accurate. Thank you for proofreading the information prior to submission.

If using a purchase order, please complete this section. Otherwise, we will simply email an online invoice to the billing contact which can be paid online with a credit card at which time the registrations will be secured. Please note that registrations are not final until either a purchase order or payment is received and confirmed. If needed, you may download our W9 via the link at the top of this form.
