  • AUTHORIZATION FOR USE OF PHOTOGRAPH OR LIKENESS:

    I do permit and authorize the University of Houston and its employees, agents, and personnel who are acting on behalf of the university to use my photograph or other likeness for purposes related to the educational mission of the university, including publicity, marketing, and promotion of the university and its various programs. I understand my photograph or likeness may be copied and distributed by means of various media, including video presentations, television, news bulletins, mail outs, billboards or signs, brochures, placement on University of Houston websites, or newspapers.

    I understand that, although the University of Houston will endeavor to use my photograph or likeness in accordance with standards of good judgement, the university cannot warranty or guarantee that any further dissemination of my photograph or likeness will be subject to university supervision or control. Accordingly, I release the university from any and all liability related to dissemination of my photograph or likeness.

    Include repeated grades on the chart and in your calculations. DO not include pluses and minuses in your GPA calculations (B+ => B). Point Value: A=4, B=3, C=2, D=1, F=0

    If you have received credit for a required prerequisite via AP, IB, or Dual Credit, note that:
    Grade Earned = "CR", Point Value = "N/A", Grade Points = "N/A", Where Taken = "AP Credit"

    In column C indicate how much you paid for that particular time. You could have paid for that one semester ($30.00) or that one year ($50.00). If you paid some other cash amount, please indicate as such.

    Column D  will tell you the remaining balance you have to pay off.

    We will review your information with the data we have and will let you know if we have any questions or concerns.

    Please note, once remaining balance has been verified and a payment has been made, all transactions non-refundable.

  • The amount you see in cell D2 is the remaining amount that needs to be paid.

  • We will look at our records and let you know, through email, the remaining balance that you will have to pay.
