  • Please Read and Acknowledge the Following:

    I (signed below) understand that associate member status is a probationary status, and that any official reference to my membership must state “Associate/Probationary Member”. Associate/Probationary members are no longer entitled to claim membership (such as in applications) in the Alpha Epsilon Delta Honor Society (and membership will not be verified,) if they are not accepted and inducted as full national members by meeting all local and national requirements. Associate membership is a leadership development program that confers probationary status at only the Texas Delta chapter. Further, I acknowledge understanding that membership fees paid to Alpha Epsilon Delta are non-refundable .

    By signing below I also acknowledge my understanding of Alpha Epsilon Delta Texas Delta Chapter's nonrefundable membership dues policy.

     

  • AUTHORIZATION FOR USE OF PHOTOGRAPH OR LIKENESS:

  • I do permit and authorize the University of Houston and its employees, agents, and personnel who are acting on behalf of the university to use my photograph or other likeness for purposes related to the educational mission of the university, including publicity, marketing, and promotion of the university and its various programs. I understand my photograph or likeness may be copied and distributed by means of various media, including video presentations, television, news bulletins, mail outs, billboards or signs, brochures, placement on University of Houston websites, or newspapers.

    I understand that, although the University of Houston will endeavor to use my photograph or likeness in accordance with standards of good judgement, the university cannot warranty or guarantee that any further dissemination of my photograph or likeness will be subject to university supervision or control. Accordingly, I release the university from any and all liability related to dissemination of my photograph or likeness.
