Airport Pickup/Drop Off Contract
*Please Note:

1) There is a $50 fee per camper for this service

2) Participants must be flying in/out of Logan International Airport, and We will only pick-up and drop-off campers on the start and end dates of each session

3) Completed contracts must be submitted at least 2 weeks prior to the start of the session

4) Only one participant allowed per form.

*Procedures:
- For arriving flights: You will receive an email 3 days prior to the camper’s arrival with the pick-up location, and name and cell phone number of the staff member who will be picking up the camper.
- We will pick your camper up from their terminal, and drive them to camp for check-in.
- Upon the camper’s arrival at camp, we will send you an email informing you that your camper is safe and settling in.
- For departing flights: We will not leave your camper at the gate unattended. If your camper is an unnacompanied minor, your airline may provide a chaperone. If not, please indicate below that your child needs a chaperone. We will provide a chaperone for an additional $50 fee.
- You will receive an email 3 days prior to the camper’s departure with the pick-up location, and name and cell phone number of the staff member who will be dropping off the camper.
- If there is an emergency that will affect a camper’s pick-up or drop-off times or location, parents and emergency contacts will be notified by phone and email.
- Campers will be transported in SSYMCA vehicles, driven by camp staff members who are trained and certified in our driving procedures.

I have read and understand all the conditions outlined on this form. I certify that my child suffers from no physical or mental condition that would affect them under these conditions. I further agree that should my child be injured by participation in this service, I will not make any claim against the YMCA relating thereto. I hereby authorize the South Shore YMCA representative to pick up my child and, if necessary, to give custody to the airline representative on the date of departure.
