Proper hair removal requires hair to be 1/4 inch in length. If hair is not the required length, hairs may be left behind. Typically 2-4 weeks of growth is required for face or body waxing. Is the hair you intend removing the proper length?

Are you taking any topical or oral medications, including antibiotics? If so, please specify.

What products do you regularly use on your skin? (What brands of lotions, scrubs, soaps, et cetera.)

Are you currently being treated for any medical conditions?

Do you agree to adhere to all safety precautions/post care including: no chemical peels, exercise, tanning, swimming, hot tubs, or intimacy for 24-48 hours after waxing?

Do you agree to come to your appointment with good hygiene? (Body wipes and tampons are always complementary and available for you to use prior to being serviced.)

Would you prefer a silent appointment? This means minimal or no conversation during the appointment. I will still review this form with you, answer any questions you have and go over aftercare instructions.

CONSENT:

It is your choice to recieve waxing services. You understand and voluntarily accept the risks associated with all waxing services. This includes: allergic reaction, skin irritation, burns, scarring, bruising, and/or soreness. You agree that this waiver is in effect for all waxing services. By signing this form, you agree to the above terms, authorizing Spellbound Skincare to retain your personal information on your private client account, and release Spellbound Skincare LLC from any liability or claims.

You agree to notify Spellbound Skincare LLC of any changes in relationship to this form.

Spellbound Skincare LLC will not treat clients with: questionable medical conditions, open wounds or sores, infections, healing incisions, et cetera.

You understand that the treatments you receive are not a substitution for medical treatment.

Defective products must be returned within 1 week of purchase.

All sales are final.

Appointments cancelled within 24 hours of scheduled appointment will be subject to a 50% fee of the entire service total. No-shows will be subject to a 100% fee. If the fee cannot be charged to the card on file, the fee must be paid in full before future services can be booked with Spellbound Skincare LLC.

For all appointments, please note that there is a grace period of 8 minutes. If you are more than 8 minutes late, rescheduling may be necessary. In the event that you can't be seen, you will be charged 50% of the service total and it will apply to your next appointment.
