Vehicle Incident, Maintenance or Accident Reporting.
- A vehicle inspection must always be done before you start to drive off.
- You as the designated driver are responsible for always doing a full and thorough vehicle inspection as per the company policy of Vehicle Safety Inspection.
- All vehicle incidents and maintenance requests must be reported below and saved on each access of the vehicle log.
- Complete the fields below - all fields are required to be completed. Please note vehicle incident and maintenance and accident reporting must be made at the same time it occurred, as GPS tracking and date-time stamps will be included to prevent fraud or misleading vehicle incident reporting.

If you have selected above that NO VEHICLE INSPECTION CHECK has been completed by you prior to driving the vehicle, you are in breach of company policy and will be responsible for costs, charges, and liabilities for all incidents, accidents, and damages to the vehicle.
It is highly recommended and is company policy that before you enter or drive in a company or rental vehicle you do a thorough vehicle inspection inside and outside the vehicle.
By indicating that you have not completed the VEHICLE INSPECTION, you will be responsible and liable for all damages, and incidents that are on the vehicle before, prior, now, or after you have driven the vehicle.
It is strongly encouraged that you complete a full and thorough vehicle safety inspection as per the company procedures found in the company FAQ Server Dabase.
