Thank you for applying to adopt a new family member with us!

Please read the application carefully and fill out completely. Note that once approved and the adoption is completed, this is also your ADOPTION CONTRACT.

Once we process your application, we will CALL you for a brief phone interview and then, hopefully, you can be approved to adopt! Please allow a day or two for us to contact you and note that we usually cannot check veterinary references on the weekend.

We almost always have kittens and cats that are ready for adoption at the Lakewood PetSmart. They can be seen there during business hours. Hours to meet kitties here in the rescue are by appointment only and upon application approval.

All of our kittens and cats are spayed/neutered, developmentally vaccinated, treated for parasites, combo tested (FIV & FeLV), and microchipped prior to adoption readiness. Most of all, incredibly LOVED by us.

This animal has been provided with vaccinations, treatments and tests appropriate for his/her individual needs and has been spayed or neutered. Documentation of these procedures have been supplied. Such documentation may have accompanied this animal when he/she came to Lakewood Kitten Rescue and is upheld as sufficient unless otherwise specified. Any medical conditions made known to LKR or developed while this animal was in the care of of LKR have been treated and/or become managed for the well-being of this animal. Any behavioral issues pertinent to this animal's well-being or his/her ability to cohabitate with other humans or companion animals has been conveyed to me to the best knowledge of LKR.

From the date of this adoption, I assume full responsibility, financially and otherwise, for the care of this companion animal. This includes any health problems, or unforeseen circumstances this animal or any companion animals in my care or acquainted with me, myself or any other humans in my family or acquainted with me, may incur from the date of this adoption. Lakewood Kitten Rescue is available for support and advice on providing the best care for any animal that has been within the care of the rescue.
