Dear Parents/Guardians, 

Thank you for expressing interest in Hope Academy for possible enrollment for the 2024-2025 school year. The following must be included with the forms attached to this letter.

• Original Certified Birth Certificate
• Student Behavior Report
• Current Immunization Form
• Most Recent Report Card

Hope Academy students are required to wear a uniform daily.  The uniform dress code is enforced.

Elementary (NO Khaki attire)

• Boys wear: white, gray, black, or burgundy shirts; gray, burgundy, or black slacks, burgundy ties, and burgundy sweaters (optional)
• Girls wear : pink, white, gray, black or burgundy blouse with collar or polo shirts, burgundy plaid slacks, skirt, jumper OR solid burgundy, black or gray slacks or skirts

Middle School (only)

• Boys wear : khaki, navy blue, or black slacks; light blue, navy blue or yellow button-down shirts or polo shirts; brown or navy blue sweaters (optional), no hoods
• Girls wear : khaki, navy blue, or black skirts or slacks (no jeggings or leggings); light blue, navy blue, or yellow button-down blouse or polo shirt; brown or navy blue sweaters (optional), no hoods
