You certify that you are responsible for your own liability insurance and have added Worcester Caribbean American Carnival Association, Inc (WCACA as an additional insured on my certificate of insurance. [If1 am a FOOD VENDOR, the "Temporary Food Vendor Application" is filled out completely and attached to this form (BELOW My intention to use or NOT use propane or a generator is clearly stated and the fee for the use of each is included.] You understand that if you fail any City of Worcester Inspections, you may not be able to participate as a vendor and will NOT qualify for a refund. You certify that you hold WCACA and the City of Worcester harmless from all liabilities incurred on the event premises. Your typed FULL NAME below confirms your agreement to all the terms of this application and the supplemental contract. Finally, you understand that if your application is NOT complete, it will NOT be processed.

Full Payment of all Vendor Fees must be made to firstname.lastname@example.org via PayPal or Zelle. Your vendor registration is not considered final until full payment is received. The event is RAIN or SHINE. Vendor fees are NON REFUNDABLE.
