  • I, * , mother/father/guardian of the following students (list names): * do understand that by accepting teams, I am agreeing to the following: (please initial each line after reading)

  • * I agree that my child will complete the ENTIRE 2022-2023 competition season including Spring Showcases. This may involve in-person and virtual classes/rehearsals.
    * I will be responsible for applicable fees (team registration fee, studio fee, prop fee, choreography fees) costume payments, and any intensives. These fees are non-refundable even if we quit teams for any reason, even if costumes have not been sewn or purchased. If I must quit, I agree that my costumes will be available for use by the replacement team member for the entire competition season. Additionally, if I drop a team after Choreography Camp, I will be responsible for paying a $50 re-staging fee. If I drop a team after 10/31/22, I will be responsible for paying the $50 re-staging fee, and all of the regional competition entry fees for the season.
    * I will respect the teacher's decisions of my child’s team placements. The amount of stage time and the dancer’s position on stage will be determined by the choreographer/instructor. Teams may involve older/younger dancers or those in higher or lower levels.
    * I understand that if there are not enough members to form a team, the team may be disbanded or combined with another team.
    * If I have questions throughout the year about the progression of my dancer or teams in general, I will address this with my child’s instructor, the Competition Director or the Artistic Director, and not with parents of other dancers.
    * I agree to stay up to date by checking the teams website regularly as well as any e-mails from DE.

  • Tuition and Fees

  • * I agree to keep my tuition account, costume and entry fee accounts up to date. I agree to submit a credit card that will be kept on file and auto-billed for any payments not paid by their due date. If I wish to pay via cash or check, I will do so prior to the due date (15th of each month) to avoid having my CC charged.
    * All accounts must be paid before my child can be entered into a competition. A late fee, which is determined by the competition, will apply if I pay past the entry deadline.
    * Any payment (or scholarship) received will automatically be applied to the oldest charges on your account.
    * If I am behind on payments, my child’s solo/duet/trio will not be entered into competition, private lessons will not be conducted, and no other charges or expenses can be put “on account.”
    * Sometimes regularly scheduled classes must be canceled due to competitions, inclement weather, etc. I understand that there are no refunds for these classes, but I am welcome to schedule make-ups.
    * If entry fees have been submitted to a competition, and my child cannot attend for any reason, there will be no refunds.

  • Attendance

  • * I will follow the detailed attendance policy which is posted on the Teams Webpage.
    * If it is not possible for my child to attend, I will notify the studio in advance out of courtesy for instructors, other team members, and desk staff.
    * I will periodically verify my child’s attendance record through the JackRabbit Portal. If there are errors, I will report them immediately.
    * If a class or rehearsal needs to be made up, I will do so as directed by the director/teacher.
    * If my child has missed too many classes, or has missed mandatory rehearsals, instructors may change his/her place in a dance, or my child may be taken out altogether. The decision will depend on the following factors:

    • Student’s prior attendance record
    • The amount of changes being made to the routine
    • If the student has mastered the routine
    • Overall cooperation and attitude 
    • Willingness to make up classes or take private lessons if necessary

    * There are deadlines for making up classes. Fall absences must be made up by the date of the Team Recital (typically in early February) in order for your child to participate. All absences from January through May must be made up by the end of May in order to be placed on teams the following year.
    * If my child is not able to attend any competition, I may be responsible for the entry fees for a substitute to take my child's place (not including Nationals), and will loan my child's costume(s) to the substitute.

  • Illness

  • * I will not send my child to in-person classes if he/she feels unwell, has a fever or any symptoms of illness. Virtual options may be available.
    * If my child has been directly exposed or tests positive for COVID-19, I will report this to Dance Etc., and agree that my child will wear a mask or quarantine as directed.

  • Injuries

  • * If my child has to miss classes due to injury, I will discuss the situation with the instructors. Each injury is different and may require a different plan of action. If the student is still allowed to compete, his/her placement in the routine may change.
    * If my child has an injury that keeps him/her from dancing for an extended period of time, he/she will not be allowed to return to rehearsals without first returning to the majority of technique classes first.
    * If a student sits out of class due an injury, he/she may still be marked absent. Actively doing physical therapy, stretching, conditioning, or taking notes may count as being present in class. The decision will be at the teacher’s discretion.
    * If a student sits out of one class, he/she will be asked to sit out the rest of the day. For example, a student that sits out of ballet class will not be permitted to participate in jazz class later.

  • Liability

  • * I agree that Dance Etc. is not liable for anyone contracting any illnesses.
    * I agree that Dance Etc. is not liable for my vehicles, even if driving to or from a Dance Etc. class or event.
    * If I have a teenage driver, I agree that he/she will not be permitted to drive themselves home during an active severe thunderstorm/tornado warning.

  • Appearance

  • * I agree that my child will attend class in the proper dress code as indicated on the teams website, including proper shoes. Socks are not considered shoes unless given permission by the instructor.
    * I agree my child will not get new piercings/tattoos, or make drastic hair color changes during competition season.

  • Competitions

  • * If my child will be missing a competition for any reason, I will notify the studio ASAP. I understand if I do not notify the studio before fees for that competition have been paid I will still be responsible for paying the entry fees.
    * I agree to arrive at competitions at least two hours ahead of a scheduled performance time (or as early as the doors open, if applicable) with my child’s make-up and hair done, dressed, fed, and ready to perform.
    * If I have forgotten any piece of my costume, he/she may not be permitted to perform.
    * Immoral behavior, use of illegal substances, foul language or being rude to staff, students, or family members at competitions is not permitted and will result in immediate dismissal. My family will act positively and appropriately anytime we are at a competition.
    * I will not call competitions directly with questions about schedule, leveling, or any other concerns. I will bring these directly to the attention of the Competition Team Director and the Artistic Director and such concerns will be handled by the studio (who regularly communicates with these competitions). Contacting competitions directly may result in dismissal.
    * If my child is in a solo, duet or trio that uses props, I understand that it is my responsibility to transport the props to the venue as well as on and off stage.

  • Volunteering

  • * I agree, as a parent, to volunteer five (5) hours of my time, per year, per dancer. If I don’t have time or ability to volunteer, I will donate $60.00 (per dancer) to the studio so that others can do the work in my place.

  • Team Recital and Spring Showcases

  • * The Team Recital is usually held annually in early February. All competitive solos/duets/trios and groups will be scheduled to perform. The main focus of our Team Recital event is competition teams. This is a mandatory event.
    * Spring Showcases are held annually in early June. There are multiple shows throughout the weekend (Friday, Saturday, & Sunday). The main focus of our Spring Showcase event is recreational classes.  Teams will perform anytime throughout the weekend; production/line may be in all shows.
    * If my child is unable to attend Spring Showcases or any mandatory rehearsals due to another commitment (graduation, vacation, award ceremony, etc.) I understand that my child may be removed from their dances/closing number or be placed in a different spot.
    * To participate in Nationals, my child must also participate in the Spring Showcases. If my child is not performing at Nationals, he/she will not be participating in Spring Showcases. The Spring Showcase is our last performance opportunity before Nationals, and the dances need to be practiced with the alternates/understudies, or choreography changes to make sure all teams and dancers are ready to perform at Nationals.

  • Perspective

  • Please list all competition dancers in your family below.

  • * If I have a concern, I will use proper channels (instructor, Competition Team Director, Artistic Director) and not talk negatively about other students/teams/ teachers/choreography to others.
    * Bullying, gossip, and negativity by dancers or parents will result in consequences (with the possibility of being dismissed from the studio).
    * Being a team member does not make me exempt from studio requirements such as team meetings, mandatory practices, hair/makeup checks, costume checks, or team obligations.
