Parking Permit & Replacement ID Form

A parking permit is required if you plan to park your car or motorcycle on campus. Motor vehicles without permits will be fined. This form is also used to request a replacement Fellow ID. Parking permits and replacement IDs are only issued to currently enrolled Fellows.
REMINDER: Fellows are required to have their AFI Fellow ID on them at all times when on AFI's campus.

Fellows are allowed one parking decal for one vehicle only. Once the request and vehicle information are received, it will be reviewed for approval. If a Fellow was previously assigned a parking decal and requests a new one, the old one will become void and reported to the Security team as such.
