Chaotic K-9 Liability Agreement

My initials indicate that I understand and agree that Chaotic K-9 Dog Training requires full payment of lessons or purchased package PRIOR to the session. I understand that packaged lessons are non refundable after the first session is completed regardless of the situation. Cancelation/rescheduling of a lesson must be done 12 hours prior to scheduled session time (certain situations excused Failure to do so will be considered as a session.

My initials indicate that I understand and agree that Chaotic K-9 Dog Training does offer various day train and board and train options. All board and train or day train packages require a $75.00 non refundable fee to secure your spot. This fee will be applied to the cost of the training session.

If booking a board and train, I give full permission for the owner and/or Chaotic K-9 trainers to care for my dog in home and in the community. I agree that my dog has NO bite history and/or aggressive behaviors. I agree that my dog is fully vetted and will provide that information to Chaotic K-9 LLC prior to drop off. Failure to meet these requirements will result in an immediate return/pick up of my dog with no refund. I agree to drop off and pick up my dog at the assigned/scheduled time. Any late pick up results in a $15.00 late fee (per 10 minutes) unless discussed and approved by the Chaotic K-9 owner. Failure to pick up your dog with an acceptable time frame and/or without notice will result in the dog being relinquished to Virginia Beach Animal Control and be deemed as abandonment. I understand that I will be notified via phone, e-mail and through emergency contact that I have provided.

My initials indicate that I understand and agree that Chaotic K-9 Dog Training does offer evaluations or consultations at a fee of $75.00. Virtual consultations are free. I also understand that refunds will not be available after the training session is completed regardless of the situation. I further understand that lesson duration is determined by the trainer at the time of service and that time spent during the session discussing my dog, dog training and/or behavior is considered training/lesson time. I further understand and agree that if my dog is aggressive, it can be prohibited from attending Chaotic K-9 Dog Training.

My initials indicate that I agree, understand, and acknowledge that the elimination or modification of behaviors is not guaranteed. The client acknowledges that dog training will not provide exact results. Each dog is different in regard to ability, breeding, and temperament. Client further agrees to accept responsibility for any damages the above-named dog may cause through malicious, aggressive or improper behavior that may occur before, during, or after all services provided by Chaotic K-9 Dog Training. Furthermore, client understands and agrees that dog training may involve risks to the client, members of the client's family, or dog. I assume all risks associated with participating in this training and will not hold Chaotic K-9 Training or its instructors responsible in the event of injury to client, client's family member or dog. I agree to comply with the instructions, rules and decisions of the training instructor as it relates to the client or dog's ability to safely complete each training session. I also agree to assume all responsibility for any damage done to property, persons, or other dogs done by client or dog's actions and will not hold Chaotic K-9 LLC responsible.

My initials indicate that I agree and understand that the above-named dog participating in the training sessions is free of any infectious disease and is current on all appropriate vaccinations, including bordatella, distemper, parvovirus and rabies. I also understand that a Health Certificate must be provided before the above named dog may participate in any training sessions. I also understand that all dogs enrolled in the training sessions must receive monthly preventative treatments for the control of fleas.

I understand that my attendance in class is not without risk to myself, my dog, other family members or guests, and other class participants (human or canine These include, without limitation, risks of physical injury, mental injury, emotional distress, trauma, death, contact with other participants and their dogs, and property damage. All risks are known and appreciated by me. I waive any and all specific notice of the existence of the risks. I shall assume responsibility for and pay my and my dog's medical and emergency expenses in the event of injury, illness, or other incapacity regardless of whether I authorized such expenses. I agree that I, as owner of the above dog(s), am solely responsible for the dog(s) behavior and physical control and agree to carefully monitor and control it at all times.

By reading, signing and dating this document, I hereby waive and release Chaotic K-9, its owner (Amberjean Gallagher), and/or owners of any or all premises on which these classes are held, and/or its employees, agents and volunteers of said owners, from any and all damages, losses, fines, claims, suits, expenses (including attorney fees and defense costs), judgments and/or liabilities of any form or nature resulting from any act or omission of any dog or dog owner participating in the classes conducted at/with Chaotic K-9 and/or under the auspices of Amberjean Gallagher. This release of liability includes, but is not limited to any injury, death, sickness or personal injury or property damage my dog or I may suffer while on or around the premises where the instruction is held. This release also includes specifically, but without limitation, any and all forms of personal injury (including death) and property damage to myself, other persons, and other animals resulting from the act or omissions of any and all dogs or dog owners and I expressly assume the risk of such damages or injuries and losses throughout the duration of the curriculum.

I also agree to defend, indemnify and hold harmless Chaotic K-9 and its owner, (Amberjean Gallagher), and/or owners of all premises on which these classes are being held, and/or the employee's agents, and volunteers of said owners from any and all claims, suits, losses, damages, expenses (including attorney fees and defense costs) judgments, fines, penalties and/or liabilities, due to any and all forms of personal injury (including death) and property damage which my pet or I may cause, or be alleged to have caused, to any person, animal, or property while on or around the premises where the instruction is carried out, or in connection with the instruction provided by Chaotic K-9, their employees, agents and/or Amberjean Gallagher.

By signing here, I understand that Chaotic K-9 LLC may cancel training services at any time with no refund. I agree that will not threaten, harm or endanger trainers. Failure to be active and involved in your dog's training may result in the end of training services (decided by trainer and owner

By signing here, I acknowledge that I have fully read this agreement in it's entirety. I agree that I have been truthful, understand the above liability agreement, are aware of the risks of training and understand the payment, refund and cancelation policies.
