Confidentiality is the preservation of privileged information. Mentors are cautioned to demonstrate professionalism, good judgment, and care at all times in handling any information related to anything discussed between the mentor and mentee. This confidentiality form will ensure that:
- Topics of a sensitive nature will NEVER be documented. Discuss in an oral format only.
- No information which could be used to identify individuals is either documented or shared.
By signing the confidentiality and non-disclosure agreement, the mentor agree that anything shared by the mentee will stay confidential.
