The undersigned acknowledges that he/she is the parent/guardian responsible for the above mentioned students tuition for the year 2024/25 and further acknowledges the receipt of the Policies for Precisions Dance Company Inc. and agrees to abide by said rules and regulations. Neither Precisions Dance Company Inc. nor its representatives shall be held responsible for any illness, injury, accident, loss or claim for damages which are caused by or arise from participation of the student/parent/guardian/sibling, family members or friends during any program, class, rehearsal, competition or other event in any facility or in any location where Precisions Dance Company Inc. is located or participating, including, but not limited to 104B Waverley Road Bowmanville. The undersigned acknowledges that there are NO REFUNDS provided on any program or extras purchased. The undersigned also acknowledges that there will be a $30 charge for all NSF cheques and pre-authorized payments. Precisions Dance Company Inc. reserves the right to use any photos, video footage or the like for publicity and marketing of Precisions Dance Company Inc.
