Please be prepared to bring in a birth certificate for your child being registered and proof of residency. The birth certificate should be the official one from the county, not the one from the hospital. If you own your home, a monthly mortgage statement or recent tax bill would be acceptable. If you rent, a lease would be acceptable. Questions about residency proof can be directed to the office at 815-942-5970. When you arrive at the school for your appointment, you will fill out some paper forms, and the rest will be completed on the computer.
