CONTRACT | The form author who has completed the demographic information above must be 18 years or older and is responsible for reviewing the following. This webform acts as a legal document, and signature and form submission are representative of understanding and agreement of all information and services provided as disclosed within. It is your responsibility to ensure that other the adults and the guardians of any minors involved understand and are in agreeance with this contract prior to providing your signature. Liability: Lauren Alyssa Photography Chicago LLC along with any representatives are released from all liability regarding property damage, any loss, personal illness or injury, emotional damages, or any damages incurred to any member participating in this photo session. Payment, Rescheduling, and Cancellations: A non-refundable retainer of $200 will be processed at the time of booking, and your remaining balance will be billed separately through an emailed invoice. Late payments of 2 weeks or greater are subject to session cancellation without retainer refund. Your session must be paid in full by 2 weeks prior to event, no exceptions. Submission of this form acts as agreement to terms of payment. No reschedules are allowed for this BLACK ROSE BOUDOIR EVENT. Cancellations will result in the amount paid being credited to a future full cost experience / session at BODY AND SOUL BOUDOIR CHICAGO which must take place within six months of the cancellation. Session Policy: No one except the hired photographer is permitted to take photos included but not limited to by use of cell phone cameras during the session unless direct permission is expressed. Only individuals being photographed are permitted to attend sessions as to eliminate risk and distractions. Your photographer retains the right to end the session at any time without refund if their safety is threatened by session participants. Your Package / Upgrades: Your guaranteed amount of digital photos and / or the option of full digital gallery is outlined in your package above. If your package does not include "Full Digital Gallery," you may upgrade your package to this option anytime prior to your session date or within 14 days of your standard digital gallery delivery. "Full Digital Gallery" pertains to any images chosen, edited, and approved for distribution by Lauren Alyssa Photography Chicago LLC, and is not to be interpreted as every individual photo taken during the session. Photo Policy: Please note that per industry standard, no RAW files or unedited images will be released to customers as photography is a creative business. All photos released are those deemed to be consistent with the quality and artistic style representative of Lauren Alyssa Photography Chicago LLC. For the same reason, filters and additional edits may not be used on any digital files you receive. Refund Policy: While Lauren Alyssa Photography Chicago LLC takes pride in delivering and backing up your precious memories, sometimes accidents or technical mishaps do occur. If for any reason your agreed upon amount of digital photos are deemed undeliverable by Lauren Alyssa Photography Chicago LLC, a full refund will be initiated within 72 hours of the undeliverable notice which would be received by email. Further compensation will not be provided and is non-negotiable. Retained Rights And Commercial Use: Commercial use of images received is not permitted without expressed permission. Lauren Alyssa Photography Chicago LLC and any representatives hereof reserve the right to share any images publicly for the use of advertising, contest entries, and for other professionally relevant purposes. Delivery Of Images: Digital images will be delivered via a private client gallery online found at www.laurenalyssaphotographychicago.mypixieset.com within 3 to 5 weeks of your session date. You will be notified when your gallery is available through email containing your web link as well as an access and download code that you may use and share with friends and family. Your client gallery will remain active for 14 days and then will be disabled / deleted from the website. During this time you may download your images at full resolution, place orders for prints, and utilize any other gallery features. Please note that during the viewing of your gallery, a watermark will be present on your images but will removed when you properly utilize the digital download feature. No watermarked images may be screenshot or shared. Downloads may be shared on social media and while tagging / photographer credit is not required, it is always appreciated!
