The first annual Scottsdale Premier Summer Camp is HERE! Come have fun and join us for the first edition of the Premier Soccer Summer Skills Day Camp. *All players are required to arrive properly dressed for soccer training (shorts, socks, indoor shoes, shin-pads)

Informed Consent and Acknowledgement I hereby give my approval for my child’s participation in any and all activities prepared by the Scottsdale Premier Soccer Soccer Camp during the selected camp. In exchange for the acceptance of said child’s candidacy by the Camp, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless the Scottsdale Premier Soccer Camp and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions. In case of injury to said child, I hereby waive all claims against the Scottsdale Premier Soccer Camp, including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including soccer. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.

Registration Details:

Camp is open to any and all entrants (limited only by number, age, birthdate).

Summer Camp Refund Policy: Upon Registration - If you cancel within 3 days before the camp start date or are a no show - there will be no refund with no exceptions.

Summer Camp Cancellation Refund Policy: In the event that a summer camp is cancelled due to unanticipated circumstances by Scottsdale Premier, participants will receive a full refund for the camp registration fees. Scottsdale Premier and Its coaching staff is not responsible for any expenses accrued in relation to the camp.

Confirmation BY AGREEING ABOVE AND SUBMITTING THIS FORM, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT IS EQUIVALENT TO AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
