Behavioural Safety - Worker Training Pack order form

Fill out the form carefully to order the materials required to train your workers in Behavioural Safety
The training pack consists of three elements:
1. An A3 easel binder which you can use to display the material to your trainees. The reverse side of the page is seen by the trainer and gives guidance on key messages.
  The trainee material is currently available in either English or Portuguese. The trainer pages are in English.
2. A short video of the talk by Tammy Fage from HSI, on the consequences of accidents. A download link will be sent to you after you submit this form.
  The video has Portuguese subtitles.
3. Individual plastic Behavioural Safety cards for each attendee showing key beliefs and expectations.
  These are currently available in either English or Portuguese.

JSC Training Material Order: Terms and Conditions

Confirmation of Order
An email acknowledging your order will be issued.

Payment
The Jersey Safety Council will issue invoices for all training material orders. Our terms of payment are strictly 7 days from the date of the invoice.

Payment can be made via BACS or by cheque. Please note that JSC are unable to accept credit card payments.

Dispatch of Materials
The training materials will be delivered to you together with details of how to download the video following receipt of payment.

Data Protection
Your personal information is collected from you when you submit this form. Your data will be handled in accordance with the Data Protection (Jersey) Law 2005 and any amendments or revisions which come into force.

Your personal information will only be used to administer the Behavioural Safety Leadership training programme.

The privacy and confidentiality of your information will be respected and will not be disclosed to any third parties without your consent unless required to by law.
