Please document your food and fluid intake in this diary for 3 separate days prior to attending your appointment with the Dietitian. Be specific, where possible. Make sure you include:
- The amount eaten (eg. 200g chicken Breast)
- The brand of the product if known (eg. Paul's Physical low fat, high Calcium milk)
- Any spreads or sauces added (eg butter, margarine, soy sauce)
Use the Comment/Notes box to record symptoms, mood or other details as requested by your Dietitian.
