Thank you for contacting The Well Centre as a private centre dedicated in the field of Psychiatry and Mental Health.
The Well Centre (TWC) treats patients at Greetwell Place, 2 Lime Kiln Way, Lincoln LN2 4US. Please note we require notice in advance for a disability friendly venue to be arranged. Please do also let your TWC booking agent know should you have a disability, which may impact your ability to read or complete forms.
GENERAL TERMS AND CONDITIONS
1. Chaperon: The Well Centre prefers to see patients with a Chaperon unless the patient declines this. Providing a Chaperon is the patient or carers responsibility. The patient must inform TWC to provide a chaperon if they cannot bring one. During the assessment, there will always be an administrator present.
2. The initial consultation will be for an hour. Mostly the consultant needs to see the patient at least twice to complete the assessment. The second session usually lasts 45 minutes to 1 hour. Normally the patient or the young person would be accompanied with the parent/s or nominated carer.
3. The fees of an initial consultation with the Consultant will be £350.00 per hour. If the appointment is longer than one hour this will be charged at £300 pro rata. Follow up appointments are charged at a minimum rate of £150 for a 30-minute session, time beyond this will be charged pro rata. If the patient requests a longer first session, the payee must inform the TWC booking agent to book additional time.
4. A deposit of 50% of the fee for the first appointment will be required at the time of booking the appointment. The payee will be given an invoice post consultation for the outstanding balance. An additional charge may be incurred if the appointment goes beyond the allotted time. Payments can be made by card on the day of the appointment, cheque, or in cash. Fees are inclusive of:
o On consent, a letter to your GP’s or Prescription (if needed)
o Further investigation request/s e.g. blood tests, MRI scans etc. GP’s and NHS services usually agree to carry out further investigations. If they decline, you may need to fund further investigations privately.
o For patients funded via charities, other terms may apply
Reports are charged on an individual basis, starting from £550, your booking agent can provide an estimate on request.
Cancellation policy: a 50% cancellation fee is payable if an appointment is cancelled between 48 to 72 hours prior to the appointment, 75% will be charged if an appointment is cancelled in less than 48 hours prior to the appointment. Cancellations made within 72 hours will not be charged.
The patient or young person could be seen 2 to 4 times. If more psychiatric intervention is required then it is usually an indication for referral to the local Mental Health Service, NHS Mental Health Services or to other mental health Professionals (e.g. Psychotherapists).
5. In The Well Centre’s capacity as a private institution, The Well Centre prefers not to issue prescriptions if that is what is agreed upon or recommended by the consultant. Most GP’s agree to start the patient on medication on The Well Centre’s recommendation. Mostly The Well Centre will follow NICE guidelines and may recommend investigations, or psychotherapy or other interventions if patient symptoms persist over time.
6. Please note that The Well Centre can provide you with some general advice and give you in depth information on Psychiatric and Mental Health matters. The Well Centre professionals can advise referral to counselling or psychotherapy and would of course create a care plan (recommendations).
7. After your consent, the initial assessment letter, may be sent to your GP or referral agent, otherwise we may send your consultation letter to you to the agreed address. Please note there maybe information in the letter that is uncomfortable to read. In addition, if there are confidential topics in relation to a patient, this information may not be shared with the carer.
8. As per General Medical Council guidelines, it is any doctor’s duty to enquire about any safeguarding issues. In some circumstances, The Well Centre will be obliged to contact third parties such as our local Safeguarding Board or Social Services for Safeguarding issues. This is rare and we will always discuss this with you first.
ASSESSMENT DETAILS
9. In addition to the above please note the following terms & conditions.
9.1. Although this assessment will be done privately, the standard of care and the quality of the assessment that is expected from The Well Centre should be no different from the professional work as an employee of the NHS.
9.2. If there are any concerns or complaints about this assessment, please try to discuss this with The Well Centre professionals first, you can however, contact your GP to discuss the matter, or the GMC directly.
9.3. For non-urgent issues, you are able to contact The Well Centre on 07921 5111 01, by calling or sending a text, alternatively you can email us on hello@thewellcentre.co.uk. For urgent medical or psychiatric emergency, you can contact the Out of Hours GP services which is connected to the Emergency Services, go directly to any A&E department in the country or contact the NHS Mental Health Crisis Resolution and Home treatment team in your residential area. You have The Well Centre’s permission to show/ share or provide a copy of The Well Centre professional’s clinical report/ or letter.
9.4. In The Well Centre’s capacity as a private clinic, our work is limited to managing patients at mild risk
9.5. If your condition is urgent and carries serious risk to yourself, others, property or if you are at risk from others
please contact NHS Crisis Services, A&E, or call 999 for immediate support.
9.6. The Well Centre as an Outpatient private service is not designed to manage patients in need of urgent care who are at serious risk to themselves or others.

10. I understand that upon completing this document I have read the terms and conditions of The Well Centre provided to me and I am agreeing with the following terms and conditions:
11. I have reviewed, read and agreed to The Well Centre Terms and Conditions.
12. I understand that The Well Centre, its affiliates and authorised agents administering the treatment (including third party administrators) will need to review, use and disclose information about me, my medical and psychiatric diagnosis and treatment, counselling, psychotherapy and e-psychiatry.
13. I understand that The Well Centre will take necessary steps to protect my privacy and information will only be shared in accordance with our procedure to enter into treatment.
14. I give permission for The Well Centre, the treating physician and other professionals to obtain and share further information from my GP, NHS and / or Consultant as needed.
15. To the best of my knowledge my physical and mental health history and status has been accurately and fully disclosed to my treating Consultant and / or Therapist.
16. It is my responsibility to inform the treating consultant and /or Therapist of any changes in my medical, psychiatric status, mental health status and social status.
17. I understand and explained that my treating physician or professionals are authorised to perform a full examination and request investigation as required.
18. I agree to enter into the treatment program and follow the required treatment schedule as discussed with the treating physician. I am aware that this may carry risks of side effects, withdrawal symptoms and other complications if I don’t comply with the instructions or I may exceed the recommended prescribed dose
19. I understand it is at my discretion to stop the treatment or investigations if I decide to do so, however I understand that:
19.1 I am aware that this may carry serious risks of side effects, withdrawal symptoms and other complications.
19.2 I have to comply with any agreed or remaining fees to be settled within the agreed period of 14 days.
20. I understand that if I decide to stop the treatment or assessment process I will be solely responsible for any further side effects to the treatment or risks due to the premature discharge against medical advice.
21. I understand that The well Centre is an outpatient consultation service, and that during the Covid 19 outbreak some of the assessment may be carried out by either telephone or utilising telecommunications such as Doxy.me.
22. The Well Centre is not designed to deal with Urgent or Risky situations (within working hours or outside working hours).
If my condition deteriorates, or if I am in an urgent or crisis condition (e.g. suicidal tendency or plan) and carries serious risk to myself, others or to property or if I am at risk from others, I shall inform my family, or carer and contact NHS Crisis Services, A&E, or call Police or Ambulance (phone number 999 or 111) for immediate support.
23. I understand and following the Health Authorities guidelines regarding COVID-19 instructions. I shall inform The Well Centre if I have any symptoms or proven infection. I shall follow the Health Authorities advice and I contact the mental health services regarding any mental health issues. I understand that The Well Centre does not treat COVID -19 related problems.
