    I give my consent to The Valley Church to use photo or video images taken of my child(ren) in church brochures, advertisements for the church, on the website, in social media, and in other church publications as they see fit. This form will apply throughout my child(ren)’s tenure at The Valley Church Vacation Bible School.

     

     

     

     

  • Liability Waiver:


    I hereby give my permission for my child(ren) to participate in The Valley Church Vacation Bible School. I assume all risks of injury, damage, or other harm or inconvenience that occurs at The Valley Church Vacation Bible School, and I agree not to hold The Valley Church, or any individual associated with the church, liable or responsible. I will also do everything possible to ensure that I or the emergency contact person can be reached at all times while my child(ren) is participating in The Valley Church Vacation Bible School.
