Thank you for submitting a nomination for the Governor’s Awards for the Arts in Ohio! We appreciate your efforts to recognize an individual or organization with one of Ohio’s highest civic honors. As the nominator, you serve as the nomination coordinator. You write the narrative, solicit support letters, and hit “Submit.”

Before You Begin
Please be prepared to submit all materials before beginning this nomination form. You cannot save and return to the form. Everything must be entered and uploaded at one time.
To complete this nomination form you will need:
1) General contact information for both the nominator and nominee (e.g. address, email, phone number). Nominees will not be contacted until winners are announced.
2) A 350-800 word nomination narrative
The narrative you submit will introduce the nominee's accomplishments and contributions.
We recommend typing the narrative in a Word document and copy/pasting your text into the nomination form.
3) Up to five support letters (500 words or less per letter)
A well-rounded nomination includes a diverse set of support letters that highlight the nominee's strengths and are not repetitive of the nomination narrative. We suggest you share the nomination narrative with the individuals you ask to write letters of support. Caution support letter writers not to restate the nomination narrative, but rather provide their own first-hand account of the nominee's contribution to the arts.
Eligibility & Selection Process
Nominations are open to organizations or individuals who work or reside in Ohio. Nominees must be living—no awards will be presented posthumously. Nominations are reviewed by Ohio Arts Council board members, and the Governor's Office determines final award winners.
