This form is for "agencies" to submit; not students.

Upon submitting this form, an invoice will be sent to the email you provide.

Enrollment is a 2 step process:

1. Enrolling - Student must enroll by selecting the class date they want, read over the class policies and sign-up with their personal information. Student should not pay.

2. Payment - By filling out this Invoice Request form, your agency agrees to pay for the student's Driver Education costs of: $390.00

Steps 1 and 2 above, MUST be done the same day to ensure proper registration.

Note: If the agency is paying with credit card, then the agency should enroll/pay HERE.
