Travel Allowance Request Form

 

Please use this form to request a travel allowance for tournaments and league games. Your travel allowance will be calculated based on this form and paid within 3 days of the trip or receipt of this completed form (whichever is later). Bend FC will provide a travel per diem for one coach per team, typically head coach unless a substitute coach is necessary. Please do not submit this form until travel has been completed.

The Bend FC travel allowance is just that, an allowance. There will be some weekends when you may feel over-compensated for your costs, and some weekends when, depending on your lodging choice, you are not. This model does not require the submission of receipts to meet IRS guidelines and provides the greatest flexibility for you. 

 

Meals will be included at the following rates:

• Breakfast & lunch are $15 each
• Dinner is $25

Mileage is calculated based on $6/gallon fuel costs and an average of 20mpg. For multi-night trips, additional miles are added for around-town travel

Lodging fluctuates depending on the time of year, and location. For Fall League 2024, the lodging budget will be as follows:

Portland/Vancouver Metro $160 per night (including taxes and fees) 

Salem/Corvallis/Albany/Eugene $175 per night (including taxes and fees)

Medford/Rogue Valley $160 per night (including taxes and fees)

Remember, it is your responsibility to stay within the budget. Sometimes the allowance may be more than your lodging expense and sometimes less. You qualify for the allowance regardless of where you choose to stay.
