Liability Release Form I understand that there are risks of physical injury associated with, arising out of, and inherent to the activities presented. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all rights and/ or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Salted Dance Compnany LLC/ Salted Performing Arts its officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities (hereinafter collectively referred to as “Salted Performing Arts or ”. I hereby agree to release Salted Dance Company LLC / Salted Performing Arts harmless of all liability and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation of the organization( the organization joining activities) on behalf of the all the participants. I am aware that this is a release of liability and an acknowledgment of the organization's voluntary and knowing assumption of the risk of injury and death. I have signed this document voluntarily and of my own free will in exchange for the privilege of participating in the organization, I am representing my organization and enrolling in the membership program. I also give Salted Performing Arts permission to use pictures of my childcare home and center in or on any form of advertisement for Salted Dance Company LLC/ Salted Performing Arts or a Salted Performing Arts affiliated event. The participant/organization has my permission to participate in Salted Dance Company LLC/ Salted Performing Arts Events. I warrant the below information is complete and correct. I further release Salted Dance Company LLC./ Salted Performing Arts of all liabilities associated with my child’s attendance at Salted Dance Company LLC./ Salted Performing Arts. *Tuition, enrollment fee, and costumes fee are nonrefundable* Salted Dance Company LLC/ Salted Performing Arts reserves the right to suspend or expel its joined organizations as it deems fit.
Membership:
I agree that membership fees are due weekly regardless of whether my facility participates that week or not. Membership fees cover only the membership fee. All other fees are additional charges.
Cancelation:
To cancel membership a 30-day notice must be given via email before the next billing cycle must be sent to Salted Performing Arts(SPA).
Organization compliance agreement:
Each organization and its participants( including organizations: students, staff, parents, and family members of students and staff must follow the guidelines and policies of Salted Dance Company LLC/ Salted Performing Arts. Failure to do so could lead to membership cancelation and denial of further participation with SPA.
SPA Cancelation:
In case of class cancelation on part of Salted Performing Arts. Salted Performing Arts will do its best to reschedule the canceled class if it is within its means to do so.
Onsite Instructors:
Onsite instructors sent by SPA are not to be left in charge of the classroom. Organizations (facility) must keep a teacher in the classroom as SPA instructor leads the class. A teacher provided by the organization must keep a qualified staff member in the room at all times.
Seasonal Concerts:
All membership holders are eligible to participate in the seasonal dance concerts. It is the responsibility of the organization to coordinate information and concert fees for its clients.
