    We are filled with so much gratitude for your interest in our church. We pray that you find what you are looking for in our sacred space. Please fill this questionnaire with honesty and transparency, it's very important that all questions be answered truthfully because working with plant medicine can be dangerous if we don't have a deep understanding of you and your health. All information is confidential and will only be discussed with you and a medical professional when necessary. If, for any reason we find that you are not be able to participate in ceremony we will discuss with you why and try to find a solution to the issue. **Please be prepared to make a $300 deposit payment at the end of this form.**
  • Contraindications:

    Our priority is your safety and well-being, and to guide you to personal healing, growth and spiritual connection. Please be as forthcoming and detailed as possible when answering the questions. If any potential medical contraindications are present, we will inquire further to ensure your safety. By registering, you are declaring that you are in physical and mental condition appropriate to the activities described. We will provide the most secure environment to work with medicinal plants as much as possible and ensure your welfare to the best of our abilities, at all times. In return, we ask that you behave responsibly and do not endanger yourself or others.

    Although plant medicines have not been found to cause psychosis or other psychiatric disturbances, it can be dangerous to those with a history of psychological conditions. Please inform us of any history of mental health conditions in the past.

    It is essential to stop taking the substances listed in this document and give your system sufficient time to remove them from the body prior to ceremony. We will provide advice on how to proceed during your booking process. Please consult your doctor if you are in any doubt. You should not stop taking prescribed medications without consulting your doctor. 

    If you are taking any drugs or herbs it is important to review the information carefully below. For your safety, all drugs and herbs listed below should be discontinued in a safe manner. Please consult your physician before changing your medical schedule.

    Herbal Contraindications for some of our medicinal plants:

    St. Johns Wort • Kava • Kratom • Ephedra • Ginseng • Yohimbe • Sinicuichi • Rhodiola Rosea • Kanna • Boswellia • Nutmeg • Scotch Broom • Licorice Root • Sassafras

    Cannabis Interactions: Smoking marijuana in combination with other plant medicines may distort, darken and/or confuse the overall experience, acting as a blockage. Given the general amplified sensitivity of all participants during the event, marijuana negatively affects not just the person who used it, but potentially other participants’ experiences as well as the overall energy of the medicine circle. It is important that your system is clear of cannabis at least 3-4 weeks prior to ceremony.

    Contraindications: Drugs and Medications 

    Any medication that contains MAO inhibitors (MAO-Is) are known to cause dangerous side effects when taken in conjunction with drinking some medicinal plants and should be suspended 
    Any medication that has an effect on the serotonin system, including Selective Serotonin Reuptake Inhibitors (SSRIs), can induce serotonin syndrome, with potentially fatal results when combined with some medicinal plants. We advise that SSRI medications be suspended SIX WEEKS prior to taking some medicinal plants to allow the system to clear in time 
     Some of these drugs may cause side effects when you stop taking them so you should allow plenty of time for these to subside and always consult your physician before changing your medical schedule.

    Potential complications that can occur while taking these listed drugs in conjunction with some medicinal plants:

    Using stimulants with MAO-Is is particularly dangerous and can be potentially fatal. 
    Using cocaine, amphetamines or MDMA (Ecstasy) with MAO-Is may cause a severe increase in blood pressure, increasing the chances for stroke and cerebral hemorrhage and making it possible to overdose on a relatively small amount of cocaine. (A fatality has been recorded involving combining Peganum harmala and cocaine. Fatalities resulting from combining amphetamines with pharmaceutical MAO-Is are recorded in medical literature.) 
    Using other serotonin agonists or precursors with an MAO-I can lead to serotonin syndrome. The main symptom of serotonin symptom may be a severe and long-lasting headache (the same symptom as MAO-I tryptamine interaction) and/or fever (as high as 40 °C / 104 °F or more) Other symptoms of serotonin syndrome may include rapid heartbeat, shivering, sweating, dilated pupils, intermittent tremor or twitching, overactive or over-responsive reflexes, hyperactive bowel sounds, high blood pressure. Severe serotonin syndrome may lead to shock, agitated delirium, muscular rigidity and high muscular tension. renal failure, seizures, and can be life-threatening. 
    Using Tricyclic antidepressants within two weeks of taking MAO-Is may cause serious side effects including sudden fever, extremely high blood pressure, convulsions, and death. 
    Using Fluoxetine (Prozac) within five weeks of taking MAO-Is may cause high fever, rigidity, high blood pressure, mental changes, confusion and hypomania. 
    Using Benzedrine, Benzphetamine, Desipramine, Desoxyn, Dexedrine, Dopamine, Ephedrine (contained in Marax, Quadrinal, and other asthma drugs), Epinephrine, 8 of 9 Guanadrel, Guanethidine, Hydralazine, Isoproterenol, L-dopa, Metaraminol, Methyldopa, Mirtazamine, Norepinephrine Oxymetazoline, Phendimetrazine, Phentermine, Phenylephrine, Phenylpropanolamine, Pseudoephedrine, Ritalin, or Venlafaxine with MAO-Is may cause a hypertensive crisis (severe spike in blood pressure). 
    Using Adderall with MAO-Is can result in high body temperature, seizure, and in some cases coma. 
    Using Bupropion (Wellbutrin) within two weeks of taking MAO-Is may cause serious side effects such as seizures. 
    Using Buspirone (Buspar) with MAO-Is may cause high blood pressure and increased sedative effects. 
    Using Carbamazepine (Tegretol) with MAO-Is may result in fever and may increase seizures, especially in epileptics. •
    Using Clomipramine with MAO-Is may cause high fever (hyperpyrexic crisis) and seizures. 
    Using CNS depressants with MAO-Is may increase the depressant effects. 
    Using Desipramine (Norpramin, Pertofrane) with MAO-Is may result in hypertensive crisis. 
    Using Dextromethorphan with MAO-Is may cause excitement, high blood pressure, and fever, or brief episodes of psychosis. 
    Using Fenfluramine with MAO-Is may result in fever (hyperpyrexic crisis). 
    Using Lithium with MAO-Is may cause fever and serotonin syndrome. 
    Using Meperidine (Demerol) with pharmaceutical MAO-Is has resulted in deaths from a single dose. 
    Using Metaproterenol or other beta-adrenergic bronchodilators with MAO-Is may cause blood pressure elevation and rapid heartbeat. 
    Using Mirtazapine (Remeron) with MAO-Is may result in hypertensive crisis. • Using Nefazodone (Serzone) may result in high fever. 
    Using Temaril with MAO-Is may increase chance of side effects. 
    Using Terfenadine with MAO-Is may cause an increase in MAO-I blood levels. 
    Using Theophylline with MAO-Is may cause rapid heartbeat and anxiety. 
    Using Trazodone (Desyrel) with MAO-Is may result in high fever. 
    Using Tryptophan or L-tryptophan with MAO-Is may cause disorientation, confusion, amnesia, delirium agitation, memory impairment, hypomanic signs, shivering. 
    Using Venlafaxine (Effexor) with MAO-Is may result in hypertensive crisis. 
    Using Ziprasidone (Geodon) with MAO-Is may cause serotonin syndrome. 
    Using alcohol with MAO-Is may cause side effects like angina (chest pain) or headaches. The headache may mask or be mistaken for hypertensive crisis caused by MAO-I interaction. MAO-Is can also increase the sedative effect of alcohol. 
    Using opiates with MAO-Is carries a risk of respiratory depression. 
    Using diuretics with MAO-Is may cause a greater drop in blood pressure than normal and increase in MAO-I blood levels. 
    Using anesthetics with MAO-Is may potentiate the anesthetic effect. 
    Using sleeping pills, tranquilizers (major or minor), or barbiturates with MAO-Is may increase the sedative effect. 9 of 9

  • Nutritional and energetic guidelines for sacred diet:

    It is extremely important to commit to preparing physically and energetically for the ceremony. It is necessary to ensure your safety and for you to receive the best results from the experience. It not only helps your body to receive the healing of Ayahuasca, but it also reduces any unnecessary purging due to excess toxins in your system. Additionally, it is a commitment, respect and devotion to the sacred medicine you are preparing to receive and to yourself. If you feel that you may need additional support concerning the diet please let us know, we are here to help.

    Four to Six weeks prior to ceremony:

    • Stop the use of any street drugs.
    • Stop the use of Cannabis.

    Two weeks prior to ceremony (and post ceremony):

    • Be mindful of the media you ingest, including anything that is meant to induce strong emotions or portrails of traumatic events (violence, stress, sadness, etc.). These energies can influence the ceremony experience.
    • No Alcohol
    • No Red or heavy meats (pork, beef, sheep, eel, etc.)

    One week prior to ceremony (post ceremony) none of the following:

    • Sexual activity or exchanges with other, including masturbation (this allows your energy to be strong, clear and purely yours).
    • Dairy products
    • Refined sugars
    • Smoked products
    • Junk food 
    • Citrus
    • Minimize salt intake.
    • Oils (only use olive, avocado or coconut oil sparingly)
    • Animal fats
    • Carbonation (soda, seltzers, energy drinks)
    • Fermented food or drinks (Kombucha, non-alcoholic beer, kimchi, sauerkraut, kefir, pickles, yogurt)
    • Aspartame
    • Chocolate or peanuts in large amounts
    • Dietary supplements and protein powder/bars

    3 days prior to ceremony (post ceremony):

    • Eat healthful meals with low levels of salt, sugar, and fat.
    • No Caffeine and other stimulants (Decaf and Green Tea is okay, in small amounts)
    • No spicy foods
    • No Fried foods

    Day of Ceremony (and day after):

    • Please eat, fasting is not recommended. Eat clean, healthy foods.
    • Simple breakfast and lunch.
    • No food after 1pm
    • Drink only water, coconut water or herbal tea after lunch.

    Post Ceremony:

    Continuing to adhere to the diet will allow the medicine to continue working, healing and aligning your system to its fullest capacity.

    For all participants involved in CAYA Sanctuary of Washington activities including but are not limited to CAYA Sanctuary retreats, ceremonies, breathwork, yoga, hiking, activities in or near flowing water and (others) on the property located at 686 Winston Creek Rd, Mossyrock, WA 98564.


    Coronavirus/COVID-19 Acknowledgement & Commitment


    I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and other public health authorities still recommend practicing social distancing. 
    I further acknowledge that the CAYA Sanctuary may invite me to be within 6 feet of other practitioners or participants. 
    I further acknowledge that the CAYA Sanctuary cannot guarantee that I will not become infected with the Coronavirus/COVID-19.
    I voluntarily seek these sessions provided by the CAYA Sanctuary and acknowledge that I may be increasing my risk of exposure to the Coronavirus/COVID-19. 
    As an adult over the age of 18 years old I take personal responsibility for my health and my participation in the CAYA Sanctuary.

    I attest that:

    I am not experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
    I have not traveled internationally within the last 14 days.
    I have not traveled to a highly impacted area within the United States of America in the last 14 days.
    I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19. 
    I have not been diagnosed with Coronavirus/COVID-19 and not yet cleared as non-contagious by state or local public health authorities.
    If after a session I learn that I have been exposed or diagnosed with Coronavirus/COVID-19 I will contact Crystal Espiritu or Damian Espiritu. 

    CAYA Sanctuary Programs

    I understand that ceremonies may include physical movement, breath, meditation, release of emotions, touch, bodywork, massage, crafting, hiking, outdoor activities, fire and sound. I understand that these modalities give an opportunity for release of chronic muscular tension and free flow of energy in the body. 
    As is the case with any physical activity, the risk of injury, even serious disabling, is always present and cannot be entirely eliminated. If I experience pain or discomfort, I will listen to my body, discontinue the activity, and ask for support from a facilitator. I assume full responsibility for any and all damages, which may incur through participation. 
    I understand that the facilitator does not diagnose illness or disease and does not prescribe medical treatment or pharmaceuticals. 
    I understand that Breathwork (and other modalities mentioned above) are not substitutes for medical care and that it is recommended that I work with my primary care physician for any condition I may have. 
    I confirm that I am not pregnant, nor have severe asthma, severe heart disease, a mental illness, epilepsy/history of seizures, acute physical injuries or severe diabetes. 
    Breathwork and other activites are not recommended under certain medical conditions. These are contraindications, to which I have been made aware. I will make the facilitator aware of any medical conditions or physical limitations before the session. 
    I am not intoxicated by alcohol or drugs. 


    Release of Liability


    I also affirm that I alone am responsible for deciding whether to participate in any CAYA Sanctuary related activities and that participation is at my own risk. I acknowledge and affirm that my participation in any on-property activities such as, but not limited to, the use of the sauna, hammocks, hot tub, lodging accomodations, river/creek and hiking is solely voluntary and I accept full responsibility for any associated risks involved and release any liability to CAYA Sanctuary for personal or physical injuries resulting from my voluntary participation in these activities.


    I hereby release and agree to hold CAYA Sanctuary and the property owners of 686 Winston Creek Rd, Mossyrock WA 98564 harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the institute, or that may otherwise arise in any way in connection with this training received from CAYA Sanctuary. I understand that this release discharges CAYA Sanctuary from any liability or claim that I, my heirs, or any personal representatives may have against the institute with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any training received from CAYA Sanctuary. This liability waiver and release extends to the institute together with all entities, properties, owners, partners, facilitators and employees associated with CAYA Sanctuary.


    I understand that this Waiver and Release agreement is binding on me, my spouse, my heirs, my executors, administrators, personal representatives and assigns. 

  • I am aware of the dangers of attending these events on these particular dates. I promise not to hold the company, its officers, agents, or employees liable or pursue legal action against them. This waiver absolves the company of any duty for injuries sustained on the premises before, during, or after the activity. By signing this agreement, I agree to hold the company completely harmless, including financial responsibility for any injuries sustained, regardless of the cause or circumstances. I will do all in my power to obey corporate staff and all safety requirements, and I will seek clarification if necessary.

  • Waiver and Release


    I, {participantName}, am an adult of sound mind and body and am more than 18 years of age. I am aware that I have elected to participate in a retreat held by CAYA Sanctuary. During the retreat, I may be drinking tea containing plants from the Amazon basin in Peru, and may be offered tobacco or other herbal medicines. I have been warned that this tea may have health and medical side effects and contraindications when taken with other medicines or supplements, and I have been told what these side effects and contraindications are. I am not taking any medicines or supplements that might interact with the tea. I have been instructed how to diet prior to this experience, and I have followed those instructions. Additionally, I know that after drinking this tea I may experience vomiting, diarrhea, stomachache, body temperature changes, crying, sweating, blood pressure changes, visionary experiences, or other side effects of which I have been warned. 


    I certify that the medical information I have given to the organizers is complete and accurate, and that I am healthy and safe to ingest the tea. I acknowledge that I do not have any physical limitations, medical ailments, physical or mental disabilities that would limit or prevent me from participating in the above-mentioned activity, and, if required, will obtain a medical examination and clearance.  If I have any such physical or mental disabilities that I am aware of, I will inform the retreat’s organizers or leaders in writing before participating in such activities.


    In addition, I agree not to hold CAYA Sanctuary, its owners, officers, employees, affiliates, members, agents, organizers, leaders, assistants, participants, legal representatives, the owners or representatives of the property we are on, or anyone else responsible for my choice to participate in the retreat, my decision to drink the tea, and any physical, mental, emotional, or spiritual effects that may occur before, during, or after the retreat, including illness, sickness, or any side effects. I waive, release, and discharge CAYA Sanctuary (and others noted above) from all liability for or by reason of any damage, loss, or injury to person and property, even injury resulting in my death. I take full responsibility for my actions. I take full responsibility for my body, health, and well-being. 


    I hereby acknowledge and agree that by signing this release I have read the information sheet and acknowledge the risks and responsibilities of my participation in the above stated activities to others and myself. I also acknowledge that by signing this release, I will be forever prevented from suing or otherwise claiming against CAYA Sanctuary (and others noted above) for any property loss or personal injury that I may sustain while participating in or preparing for the above noted activity. I have been given the opportunity and have been encouraged to seek independent legal advice prior to signing this Waiver and Release agreement.


    I understand that this Waiver and Release agreement is binding on me, my spouse, my heirs, my executors, administrators, personal representatives and assigns.

  • This membership is an acknowledgement of your personal commitment to yourself and the other members of the church. Membership also includes the commitment from the staff, facilitators and volunteers at CAYA Sanctuary to uphold all members safety and security, as well as continued support before, during and after ceremony. The following are agreements and guidelines for becoming a member of the church.

    CAYA Sanctuary is a Spiritual Community, which evolves through individual and collective healing experiences, personal growth work, and medicine ceremonies. Core to the evolution of CAYA Sanctuary is sacramental plant medicine ceremony. Ayahuasca is its central sacrament to the community’s spirituality and ceremonial practice.


    CAYA Sanctuary Medicine Ceremony is a spiritual group process through which people commune in ceremony with ayahuasca. This work evolved directly from the lineage of traditions that have held ayahuasca as a central sacrament and a crucial catalyst to personal and collective spiritual connection and evolution.


    History and Lineage


    CAYA Sanctuary was born from the desire to create a church for people to come together with the shared interest of expanding the spiritual connection and healing from the energetic impact of trauma which impedes us from embodying our divine nature. 


    CAYA Sanctuary is here, first and foremost, to support the complete connection to ourselves, our ancestors, and to the Divine Spirit of the Creator. CAYA Sanctuary’s mission is to co-create a sacred space of love, compassion, forgiveness and unity for all humanity so we may see each other as a collective, working together in harmony. Traumas endured in this life can, and often do, disconnect us from ourselves, others and with the spiritual aspects of life. This disconnection magnifies the importance of healing the mental, emotional, physical and spiritual parts of our being and to bring them into coherence. When mind and spirit are out of harmony it causes great suffering in our life, when mind and spirit are in harmony we experience peace, comfort and wellbeing, proving the inseparable nature of healing and spirituality. Through ceremonies, service to others, applying ancient spiritual teachings and community engagement, we can restore our connection to Divine Love, which is the birth right of all beings. 

    In order to be in alignment with the sacred spirit of Ayahuasca, CAYA Sanctuary’s commitment is to follow the South American Amazonian shamanic lineages, such as the Shipibo. CAYA Sanctuary is dedicated to learning from, encompassing, and embodying the ceremonial traditions of honoring Ayahuasca as the Indigenous people have done for thousands of years. In order to best serve and accommodate individual needs, within a modern context, we also utilize other holistic healing and spiritual modalities.


    The founders of CAYA Sanctuary


    Crystal worked extensively with the Indigenous communities of the Pacific Northwest concerning the impact of individual and generational trauma on humanity's connection to their roots in a spiritual existence. As a result of this work and the immersion into the spiritual and esoteric aspects of ancient indigenous wisdom Crystal was inspired to bridge the gap between spirituality and healing. This bridge has manifested in the form of a community based church, known as CAYA Sanctuary, offering holistic healing through immersion into our spiritual connection, indigenous wisdom, ceremony and faith. To expand the potential of healing there needs to be ample time, space and a dedicated support system. Crystal’s understanding of the connection between healing and spirituality became crystallized after her first Ayahuasca ceremony. The days, weeks and months following the ceremony she experienced a deep understanding of the spiritual nature of the healing provided by Ayahuasca and consequently had a transformational experience. Crystal has participated in ceremonies across multiple countries in Central and South America experiencing different traditions all with profound healing and wisdom. With her background in mental health and knowing the dire situation concerning humanity’s disconnection and suffering, she and her beloved partner dedicated a church that could safely and effectively give people the opportunity to commune with the sacred spirit of Ayahuasca. 


    Damian has a deep understanding of trauma and addiction having grown up in a family where it (was a near daily experience) had been very prevalent. He has witnessed firsthand the impact that trauma and addiction have on a person’s life, causing suffering, dysregulation and often more trauma. He is dedicated to helping anyone willing to overcome their limitations and free themselves from the cycle of trauma. It wasn't until Damian's first ceremony with Ayahuasca that he was able to see how unconsciously he had approached life. For the first time, he realized he has a genuine choice in deciding how to respond to life. Since his first transformational experience he has dedicated his life to serving others by creating space for connection and healing. Damian used the skills gained while working in construction to help build CAYA and transform a dream into reality. Damian leads integration groups where he provides a safe container for people to heal by processing their challenges and incorporating lessons from ceremony into daily life. Damian spent time living and working with a tribe in the Amazon of Brazil, not only learning about sacred plant sacraments, but also building structures and tending to the land. He believes that to truly know the culture it’s best to experience their existence as authentically as possible, meaning, getting to work side by side with them digging holes and sweating it out together. Damian brings a deeply grounded connection to CAYA, whether in Ceremony, Integration or day to day logistics. He understands the importance of stepping into the mystical/healing realms and grounding the teachings into our daily lives. Damian continues to learn from his teachers and guides from many traditions and will continue to work towards a deeper understanding of the healing traditions of the Americas.


    New Revelation and the Evolution of Spiritual and Ceremonial Communing


    CAYA Sanctuary is itself evolving and maturing through Divine inspiration and collective consensus - towards improving the effectiveness of its ceremonies and its organization in promoting love, peace, light, joy, awareness, integrity, truth, and harmony within each person, in the CAYA Sanctuary community. CAYA Sanctuary also integrates other indigenous medicine cultures and wisdom traditions to allow a more universal scope that welcomes the participation of those with any other religious or spiritual identity. The ceremony and its songs express the group's spirituality through harmony, beauty, prayer, gratitude, as well as respect for the mysterious complexity that we find in each of our lives and the apparent contradictions and imperfections of our living experience, and also to build faith in the ultimate unity, meaning and harmony of all existence.


    Ayahuasca intensifies our process of connection and also of self-understanding. The group and ceremony structure, with dedicated helpers and support staff, provides a safe and stable environment for people to let down their guard and defenses - opening each person to their inner teacher and inner guide.


    In signing here, I explicitly acknowledge my membership in CAYA Sanctuary spiritual community, and that my participation in its ceremonies and taking its central sacrament, ayahuasca, is a spiritual and religious act. My actions are spiritual and for the purpose of supporting and connecting with my local and global human community, towards a life in sustainable integration with nature and the earth, and in appreciation of all creation and the One source of all.


    My prayer, celebration, healing and personal reflection comes through this medicine. Ayahuasca is central and crucial to this ceremony and the practice of my spirituality.

    If this is my first time, I come to experience and to survey the potential of this as a personal path – whether regular or occasional. 


    If I am returning to this ceremony community, it is because I see this ceremony and its community as an important part of my spiritual life.
