Acknowlegement of non-refundable booking deposits
In order to reduce no-shows and ensure that we maximize appointments times for our patients in need, we require a non-refundable booking deposit of $60 for all new clients in order to schedule their first appointment. The booking deposit is not an additional fee: it is applied to the total of your visit, and any remainder is due at check out. The deposit is non-refundable: it is kept as a missed appointment fee if the client no-shows, cancels, or reschedules less than 1 business day in advance of the appointment time, and a new deposit is required for the next appointment. The deposit is transferrable to an appointment that is rescheduled at least 1 business day in advance.
The $60 booking deposit will also be made required for established clients that no-show a scheduled appointment. Repeat no-shows will require permanent prepayment in order to schedule.
There is a minimum $100 non-refundable booking deposit for all surgery appointments: this will also be applied to the visit total, or kept as a missed appointment fee if the client no-shows, cancels, or reschedules less than 24 hours before the scheduled appointment time. The deposit is transferrable to a surgery appointment that is rescheduled at least 24 hours in advance.
Due to the CareCredit terms of service, CareCredit can not be used for booking or treatment deposits deposits, but at check out we can refund the deposit to the original form of payment and add it to the total for a single CareCredit bill.

Care & Payment Policy
Our clinic requires that payment be made in full at the time of service. We accept payment by cash, approved personal checks, Visa, Mastercard, Discover, American Express, Apple Pay, Samsung Pay, and CareCredit.
Our clinic does not provide in-house billing: our recommended payment plan is CareCredit, which is a medical credit card that allows the user to pay their bill in installments over a 6 month period with no additional interest as long as the CareCredit terms are met. Clients that do not qualify for CareCredit are encouraged to secure funding from a licensed and insured money lender such as a bank, credit union, or credentialed short-term loan company prior to committing to a treatment plan, as all payment is due at time of service.
Our clinic does not accept any pet insurance directly: clients utilizing pet insurance are to pay their bill in full at the time of service and then submit a claim against their policy for reimbursement. We are happy to provide any documentation requested by a pet insurance company or client to process a claim.
Our Doctors will develop a detailed treatment estimate based on your pet’s health condition, status at the time of exam, and anticipated treatment needs. The estimate will be reviewed with you to ensure you understand all aspects of the treatment plan. Though we will make every effort to stay within the range specified on the estimate, your signature below indicates you understand that charges may exceed the estimate terms, and acknowledge that you take full financial responsibility for any and all services.
For pets admitted to the hospital for treatment or surgery, a treatment deposit of 50% of the upper end of the estimate is required before treatment will be initiated, except for CareCredit users: CareCredit users will be required to leave a cash or non-CareCredit deposit equal to any amount over their available credit to equal the upper amount of the estimate. For example, a CareCredit user with $1000 available credit with an upper estimate amount of $1250 will be required to leave a $250 deposit. For all clients, the balance remaining is due at discharge by a payment method listed above: patients will not be released until payment is made. Any unused portion of a treatment deposit will be refunded.
Our clinic will send statements for any accounts with an amount due on a monthly basis: each statement processed will incur a $2.00 statement handling fee and 1.5% interest on any amount due over 30 days. Unpaid balances will be sent to a licensed and bonded collections agency between 60 and 90 days overdue.

Staff and Client Safety Policy
We are committed to providing a safe environment for our doctors, staff, clients, and pets.
Our policy is zero tolerance for profanity, physical or verbal threat, intimidation, and animal abuse. Any client interacting with our team members, other clients, or pets in any manner deemed unsafe or aggressive will be ordered to leave the premises at once, and all team members are authorized to contact law enforcement as they see fit. We reserve the right to refuse service to any person or pet.
If clients feel they are in an unsafe situation with another client or pet, we encourage them to notify our staff immediately.
In order to keep our team members, your pet, and yourself safe, as well as allow us to keep pet discomfort to a minimum, we ask that you allow our team members to restrain your pet during all procedures and treatments, and follow all instructions from the doctors and staff while they are working with your pet. All clients have a right to decline a procedure or treatment for their pet; however, please understand that doing so may mean that we can not proceed with additional treatments, procedures, or prescriptions.
Our clinic is considered a medical facility: children under the age of 18 must be accompanied and supervised by a parent or guardian. Consent to medical treatment of a pet must be given by a client or authorized representative over the age of 18. We agree that children should participate in pet ownership, but when it comes to legal requirements, we must abide by the consent of the person of legal age.
