Note: We provide T-shirts for U6 division only (included).Uniform cost is not included in the registration fee. This will be paid to your coach separately.
Players will need cleats, shin guards and the correct size ball for their age division. U6-U8 Size 3, U9-U12 Size 4, U13+ Size 5Shin guards are absolutely required for games. Sock must be on the outside and completely cover the shin guard.

Liability Release
I, the parent or legal guardian of this player, a minor, agree that I and the player will abide by the rules and regulations of the United States Sports Specialty Association. I, for myself and the player and our respective heirs, administrators and successors, intending to be legally bound, hereby release and indemnify the (USSSA) the owners and operators of the facilities used for the Programs, and their respective directors, officers, employees, agents and representatives from and against all claims, liabilities, damages or causes of action arising out of or in connection with the player's participation in the programs including, without limitation, player's transportation to/from any program, which transportation is hereby authorized. I future grant USSSA and Soccer Association of Independence, the right to use the player's name, picture and/or likeness in printed broadcast and other material concerning the Programs provided such use is related to the player's status as a participant in the Program.
Earrings
Persuant with Law 4 of the FIFA Laws of the Game (below), I understand that earrings (or any pierced jewelry) are NOT allowed in the game of soccer. Earrings and other jewelry MUST be removed for the duration of the game in order to participate. This includes recent piercings. Bandaids, tape or other remedies of protection will not be allowed.
“A player must not use equipment or wear anything that is dangerous [to themselves or to other players]. All items of jewelry (necklaces, rings, bracelets, earrings, leather bands, rubber bands, etc.) are forbidden and must be removed. Using tape to cover jewelry is not permitted. The players must be inspected before the start of the match and substitutes before they enter the field of play. If a player is wearing or using unauthorized/dangerous equipment or jewelry the referee must order the player to: remove the item, leave the field of play at the next stoppage if the player is unable or unwilling to comply."
The following are prohibited from any soccer fields, including games and practices, and will result in being asked to remove these things from the playing area, or in some cases from the facility.

Parent Behavior & Expectations
I understand that as a spectator, a referee may stop the game, or I can be asked to leave the park, if my behavior is not positive and respectful to coaches, refs, players, other spectators or league officials. My refusal to cooperate may result in a forced forfeit for the team.
