This form serves as the OFFICIAL ORDER FORM for LSM K-Shop ITZY 'CHECKMATE' Album (Sealed Album with JYP POB) Takers.
Kindly fill up this form accurately and completely.

LSM K-SHOP TERMS AND CONDITIONS:

 1. Filling up and submitting this form means that you are a 100% sure ordering. Strictly NO CANCELLATION of orders. We will be posting now BOGUS BUYERS unless you have paid for the cancellation fee of 50% of the said ordered item/s price.

2. Kindly answer the form completely and accurately to prevent any issues regarding order details and buyer's details. LSM K-Shop will not be liable for any wrong information/details that buyer had filled up. PLEASE COMPLY.

3. All personal information will be kept with utmost confidential and will be use only for this PH GO. (excluding: Initial of names/usernames, Mode of Payment (MOP) which will be input in the Shop Masterlist)

4. Ordering will be unlimited until out of stock and as long as you pay on/or before the given DOP date and time. Late payment fee will be automatically add 50 pesos per day once you have failed to pay on the given DOP.

5. All payments are NON-REFUNDABLE unless supplier failed to secure the ordered item/s or if it shows no possibility of restocking.

6. ESTIMATED TIME OF ARRIVAL (ETA) means that this isnt accurate at all. This may due to international couriers delay under their some circumstances and which LSM K-Shop are not liable enough. Please be informed as well that we need to wait for more albums ordered to arrive to save International Shipping Fee (ISF).

* Normal ETA: 4-6 weeks estimated date of arrival to PH Supplier's Address

* Fast ETA: 1-3 weeks estimated date of arrival to PH Supplier's Address

7. Packaging Fee are not yet included on the item/s price. This will depend on the size and weight of the said item/s. This will be 1:1 ratio which means that 1 item will be packed and paid as 1 as well. 

8. LSM K-Shop will not be liable for any damage/s and loss caused by International Shipping and local shipping. Actual proof of evidence (photo or video) will be announce thru LSM K-Shop Update account (@LSM_Updates) upon receiving of the item/s.

9. Defects or incomplete inclusions caused by manufacturer/s are not control by our shop and will not be responsile for it. (We already encountered these concerns before and WILL NOT be liable anymore if this happens again.)

10. All updates will be posted thru our twitter account @LSM_Updates and with the use of our Official masterlist as well. Please check it from time to time as we will update the said links with the latest updates.

REMINDERS:

All album version are under pooling. This is because we cannot guaranteed the versions that we will get as this will be bought to different sites. Pooling will be based on first-to-pay basis.

 

CLARIFICATIONS/ FAQ (Frequently Asked Questions)

 1. Can I still choose my desired album version?   -Yes, you may still choose your desire album version but again this will be first to pay basis. 

2. Are all albums will be Sealed when deliver to my address?   -Yes, all album will be sealed.

3. How can I reserve my slot to be prioritize in pooling?   -You should pay 100% of your album/s ordered to us to have a secure slot and to be prioritize in pooling order.

4. I will avail the Set album versions, is it 100% sure that I will get all album versions?   -All Set takers are prioritize when it comes to getting all versions. You will be automatically on top of the pooling order once you ordered the set albums. Hence, this isnt a 100% sure that you'll get all versions and will still based on the versions that we will be getting.

5. How will the albums be pack?   -Packaging for the albums will include corrugated box, bubble wrap for the album, bubble wrap outside the box. The poster tube arent included yet but can be requested thru DM or email once it will be deliver to our KR Address as rolled.

6. Is there a circumstances that albums are delayed? -Unfortunately, Yes. This is due to different reasons which we cannot control, but still we will assure and update you all from time to time.
