Please fill out the following information to order your custom rabies tags.
PLEASE REVIEW THE BELOW IMPORTANT INFORMATION BEFORE ORDERING YOUR TAGS!
- Your Penn Vet account number is required to place an order for customized tags. If you do not know your account number click here to find it in your user profile information.
- Next year's custom rabies tags (Orange Oval) normally take approximately 2 weeks for processing. All other custom rabies tags and orders placed during peak ordering time (September through December) may have a longer processing time. Rabies Vacc will be imprinted on the tags.
- When entering the information you want on your tag, please keep in mind that the character limits are maximums and spaces count as characters. Due to the narrow shape of oval tags it is HIGHLY recommended you enter your phone number as the 1st line of engraving on your tags.
- The engraved font size varies depending upon the number of characters used per line. The manufacturer may choose to use an additional line or abbreviate reasonably to provide the best possible result.
- Customized rabies tag orders are drop shipped from the manufacturer and incur a shipping fee. The shipping charge is determined at the time of shipping and will be included on your invoice.
- You will receive an order confirmation email upon submitting your form at the email address you provide below. Please keep this for reference.

This service provides an additional layer of protection when a pet is lost. Registration costs $2.99 and can be done by the vet practice or the pet owner. If the pet gets lost the finder can easily scan the QR code on the back of the tag with a smart phone for quick access to the owner's contact information.
