PROGRAM DETAILS
Maggio Multicultural Academy is a hybrid school experience giving our students the ability to interact and team up with other students around the world. The shift is from teacher centered to learner focused, with a mix of independent and collaborative learning, that is personalized, applicable and relevant to what students need today. We work relentlessly to ensure our students have the necessary skills and practical experience to be successful in any industry.
Which Program Will Your Child Be Enrolling In:*

Elementary: Grades 1-5 - $2800/yearMiddle School: Grades 6-8 - $3000/yearHigh School Grades 9-12 - $4000Credit Recovery - $500 / course

You will be sent an invoice for payments after submitting this form.
Upon submitting this contract you will be required to pay 1 full payments OR if choosing payments, in 2 payments. Your child will not be registered until payments are made.*

I understand that choosing to make payments, I am liable for said payments whether I withdraw my child for any reason or my child is dismissed due to inability to follow policies and regulations contained in our school handbook.*

PLEASE READ THE FOLLOWING CAREFULLY BEFORE SIGNING ACKNOWLEDGEMENT AND RELEASE AUTHORIZATION FOR MEDICAL TREATMENT:
I am familiar with the program for which my child is registering. I understand that this program involves some parental support, especially for Elementary grade levels, as it is a homeschool/self-paced/independent program. I am also aware that my child is to be present for any Live virtual meetings whether academic or social in nature. I also understand that if my child is in the middle school or high school program, they are required to attend all Live mentoring and entrepreneur mastermind sessions unless I am in a time zone that does not allow my child to participate, which he or she will then receive the recorded session.*

High School Students ONLY: I understand that my child is required to do 15 hours of community service each year, and the community service must be approved by my child's advisor or school administrator. I am also aware that my child can speak to his or her academic advisor for help on ideas or ways to do community service if he or she needs help finding a meaningful experience.*

MEDIA/ARTWORK AUTHORIZATION:
I agree that any photographs/digital images/videos taken by Maggio Multicultural Virtual Academy during Live sessions of my child as a program participant, and copies of artwork or classwork, shall be the property of MMA, and may be used by MMVA, at its discretion, for any publicity, education, marketing and/or advertising purposes and I hereby consent to and authorize such use without restriction. Students will only be identified by first names in any and all press campaigns.
