This form is meant to accomodate all types of events and not all fields will pertain to your event.

NOTE: All forms will include a dropdown of current teacher members as only their students will be allowed to participate in events. Additionally the SMAC insurance waivers will be on every form and there will be NO exceptions to the Photo & Video wavier.
