• Event Registration Form Creation Request

    Use this form to request the SMAC Webmaster create or modify an Event Registration form for your upcoming event.

    ALL events require an on-line registration form.

    This form is meant to accomodate all types of events and not all fields will pertain to your event.  

  • NOTE   Please allow at least ONE WEEK for your request to be completed.  We will try to accomodate emergencies but cannot guarantee time will permit.

    The Webmaster will contact you with questions, if any.  If they believe a Zoom meeting would be beneficial to discuss your needs, you will be contacted to schedule one at your convienence.
