    Event and Fundraising Applications must be approved before completing this form.

  • NOTE:  All forms will include a dropdown of current members. If the event is open to non-members and students of non-members for an additional fee, there will be a space to fill in the teacher's name and contact information. Additionally the SMAC insurance waivers will be on every form and there will be NO exceptions to the Photo & Video wavier.
