*, understand that Skylite Houseboat Management LLC. must secure a credit/debit card before they can schedule any requested services.

Please read and initial as follows;
*I understand that payment for the requested Turn Service is due immediately upon completion and will be charged to my card on file.
* I understand there is a mandatory 5% National Park Fee. Additionally, I understand there is a 5% credit card processing fee if I use a credit card (only applies to turn service, fuel is paid directly at the fuel dock with your credit card unless the card declines) Also, If I choose to pay by check to avoid the processing fees, I recognize that the check must be delivered to Skylite Houseboat Management prior to my houseboat departure and that it will be not be cashed until the services is fully preformed.
* I understand the card I put on file will be charged for my fuel and services. I also understand I will be charged a fuel surcharge of 5% of the Fuel if my card declines at the fuel dock and the company card has to be used.
*I understand that if my invoice is not paid within 10 days, a 20% late fee will be added to the invoice and will be automatically be charged to the card on file, unless there is a dispute on the said invoice. In which case, the Manager/Owner shall provide the basis for said dispute in writing within TEN (10) day of receiving said invoice
* I understand that Wahweap Marina and National Park Service does not allow any companies (working within the marina/park) to throw away trash within the park, and if I leave trash on the houseboat, then I may be subject to additional fees.
*I understand that if we request to be piloted in and we are more than 15 minutes late to the Marina, and I did not communicate with Skylite Houseboat Management LLC. ahead of time, then I may be subject to additional late fees.
*I understand that if we are requesting pilot services, due to the monsoon season, our pilot in/out cut-off time is 12 pm, unless the case of an emergency.
