  • NEW for 2026: W-9 Form only required for people who receive payments of $2000 or more from MEF in 2026.

    If you are working Band Camp, please describe your technology needs (including computer access, projector, WiFi, speaker/audio needs, etc.) Please limit to only what is essential as we only have one on-site coordinator for the entire camp.
