I have contacted the advisors or coaches at the Center forHealth & Financial Solutions, Inc(CHFS) of my own volition for the purposeof obtaining information and/orservices solely for success coaching, health & wellness coaching, personalfinance coaching, and any other coaching purposes.

I understand that thetele-coaching session is NOT a therapy session. Therefore, I release any advisors,coaches representing CHFS and Center for Health & Financial Solutions Inc. from any and all liability for any claims for damages that I might incur as aresult of this consultation, including claims for emotional, mental, physical,or financial damages.I understand that all tele-coaching services at CHFS are foradvisement, coaching, educational, motivational, and inspirational purposesonly. Suppose I should experience any positive emotional, psychological,mental, and physical benefits as an added benefit. In that case, I understandthat it is not any of our advisors or coaches' intention to serve as atherapist, psychologist, psychotherapist, psychoanalyst, sex therapist,addiction therapist, marriage counselor, or member of the clergy and that CHFSand its staff has never represented themselves in this capacity to me. I alsounderstand that no therapy–psychological, marital, addiction, sex, orcommunication therapy–will be done at any time, nor will there be any formalevaluations. This Release and Waiver will apply to any and all follow-up orsubsequent telephone calls or e-mail sessions I should have with CHFS and itsstaff. I also understand that all e-mail or telephone communications will becompletely personal and confidential.If any legal actions should occur, all of CHFS and its staff'sattorney fees and any additional costs will be paid for by me.
