Transportation and Activities

I/we understand that students will be transported in approved vehicles to and from Living Waters Camp and Avonhurst Church. Drivers of the vehicles will be at least 21 years of age and have a valid driver’s license. Staff certified in First-Aid and CPR will be available throughout the week.

I/we recognize that travel, and camp activities, can have inherent risks that must be assumed to participate in this experience. The inherent risks may arise from foreseeable and unforeseeable dangers, hazards, and other causes. Participation in Youth Camp may result in unforeseeable injury, illness or other damage.

Knowing and recognizing these facts, I/we choose to attend Youth Camp and the included activities / allow my child to do so, and expressly assume all of the risks that such activity may present. As required by Avonhurst Church prior to allowing me to participate in this activity, I/we waive my/our rights to pursue or
maintain any and all claims, suits, and demands, actions, causes of action, or recovery of any kind whatsoever against any activity sponsor, including without limitation, Avonhurst Church, it's Staff and Volunteers, and those of Living Waters Camp.

I/we freely choose to participate in all Youth Camp activities, accepting the risk and responsibility upon myself.

Agreement Details:

1) I agree to obey all leaders and authorities at all times.

2) I agree to listen to, and follow, all safety guidelines at all time.

3) I agree to act responsibly at all time, during both scheduled activities and free time.

4) I agree to forfeit any and all opportunties the Camp offers if my behavior becomes inappropriate or dangerous.

5) I agree that any innappropriate and dangerous behavior will lead to my expulsion from the Camp, without refund.

6) I agree to forfeit my rights to legal pursuit of any claims regarding injury or loss following my free decision to join this Camp and the Activities.
