This form is used by current and former clients to request paper copies of statements or other documents to be mailed to them by physical mail instead of the standard email delivery.

Please note that each form can only request a single document (for example, one month's statement). If you are needing statements for a longer period of time, such as multiple months, then we suggest requesting a yearly statement instead so that it includes all information you need and you only have to pay for one document to be sent.
