I,    First Name   Last Name (“debtor”),

with ID / Passport Number

hereby request to loan a car from GemachSA and that the car will be collected at Unit 4 Fairwood Estate and returned on the specified date failing which I will be liable for a late penalty of R750 per day.

I also confirm that I am aware that I will be liable to the car excess of R3 000 should I have an accident chv"s. Tyres, small scratches and windscreen damage will not be covered by the excess and I confirm that I will personally fix any tyre, electrical, small scratches and dents.

I confirm that I will be the ONLY driver of the car and will drive carefully and responsibly.

I confirm that I will return the car in the same condition as received which includes a full tank of petrol (assuming car will be full on collection) and cleaned.

I confirm that I will disclose any knock, dent or any accident whatsoever, and immediately notify office@torahplatinum.co.za on the day that it occurs.

I confirm that I will drive within the speed limits and will be liable for any traffic fines.

I confirm that this car is for in-town usage and may not be used for trips more than 1 hour outside of JHB.
