Parent/Guardian's Agreement

I, the parent/guardian of the registrant, (name of athlete), a minor, agree that the registrant and I will abide by the rules of the Mondo International Football Academy. I guarantee that my child is physically fit to undergo the different activities of the academy. Having considered the benefits that my child will derive and recognizing the possibility of physical injury associated with the sport and in consideration for the academy accepting the registrant for its soccer programs and activities. I hereby release, discharge, and/or otherwise indemnify MIFA, their coaches, and associated personnel, including the owners of the fields and facilities utilized for the programs, against any claim by or on behalf of the registrant as a result of the registrant's participation in the programs.

I authorize the use of photo/video taken of my child during this program for Mondo International Football Academy materials. I agree that upon the huge investment of Mondo International Football Academy in my son/daughter's football and academic development he/she will not be involved in any form of registration with another club/academy without a written clearance from Mondo International Football Academy.
