    Please thoroughly read the Terms & Conditions before booking your appointment. By neglecting to do so and showing up with any of the conditions that makes you an unsuitable client. We will refuse the service and you will forfeit your deposit.

    ** PLEASE READ THOROUGHLY READ THE POLICIES AS IT CONTAINS IMPORTANT INFORMATION FOR YOUR APPOINTMENT **
    PAYMENTS:
    • The remaining balance is due in cash (PREFERRED), card, venmo, zelle, cashapp.

    NON-REFUNDABLE DEPOSIT:
    • A non-refundable deposit is required to secure your date and time.
    • The deposit is added towards the total service price.
    •The deposit is non-transferable to another client or service. No exceptions.

    CANCELLATIONS & RESCHEDULING:
    • All clients can reschedule an appointment online up to 72 hours before the appointment.
    • Clients may reschedule up to two (2) times.
    • Text Alexandra if you need to reschedule within the 72-hour time frame of your appointment.
    • If you reschedule within the 72-hour time frame, the deposit will be forfeited, and another deposit is required to schedule another appointment.
    • Text Alexandra if you would like to cancel your appointment. Cancellations cannot be made online.
    • If you cancel your appointment, your deposit will be forfeited, and a new deposit is required for a future appointment.
    • Late cancellations within 24 hours of the appointment are subject to a credit card charge of 50% of the service price before any promotion application. No exceptions.
    • Emergency cancellations/reschedules require proof to waive the fees listed above. No exceptions.

    TARDY POLICY & NO-SHOWS:
    • Clients who are more than 15 minutes late must reschedule and must submit a new deposit to reserve a new appointment.
    • Any "no-call, no-shows" are subject to a credit card charge of 100% of the service price before any promotion application.

    COVID-19 PROTOCOLS:
    • Guests and children are not allowed on the premises unless they are a translator.
    • Masks are required for all guests, both vaccinated and unvaccinated with the COVID-19 vaccine.
    • Please reschedule your appointment if you are feeling sick. Proof of a COVID test, either positive or negative results, is required to transfer your deposit to a new rescheduled date.

    DISCLAIMER:
    • We reserve the right to refuse service.
    • Prices are subject to change at any time.
    • We cannot 100% guarantee the outcome of your permanent makeup procedure as they will fade anywhere from 20%-50% or more depending on the client's skin type or failure to follow aftercare correctly.
    • This service is a two-step process and requires a touch-up 6-8 weeks after the initial appointment. The second session will perfect and achieve your desired results. In very rare cases, clients will need a third session due to having a skin type that is more resistant to ink and will be priced accordingly.

    Please note that when healing, you must avoid the gym, sauna, sun, swimming until you are fully healed. Healing takes up to 2 weeks for brows , in very rare cases, longer. I advise that you schedule your appointment accordingly around any special events and vacations. ♡

    Health Contraindications for Permanent Makeup:
    - You are under 18 years of age. Parent/guardian consent is not accepted.
    - You are pregnant and/or breastfeeding.
    - You received Botox or fillers 6 months prior to the service date anywhere on the upper half of the face (eye level and above).
    - You have glaucoma.
    - You have diabetes mellitus.
    - You have high blood pressure or mitral valve disorder.
    - You have transmittable blood diseases (i.e. Hepatitis B/C, HIV/AIDS, Tuberculosis, Malaria, Syphilis, etc).
    - You have an autoimmune disease.
    - You have cancer and/or are currently under or scheduled for radiotherapy or chemotherapy treatment, or received treatment within the last 6 months.
    - You take mandatory blood-thinning medications daily.
    - You are a hemophiliac.
    - You have skin conditions on or near the procedure including, but not limited to: eczema, psoriasis, scabs, cuts, cold sores, shingles, undiagnosed rashes, or blisters.
    - You have moderate to severe acne on or near the procedure area.
    - You have experienced post-inflammatory hyperpigmentation.
    - You are allergic to pigmentation and makeup.
    - You have healing disorders.
    - You are prone to keloids.
    - You are taking prescribed skin medications to cure acne (i.e. Accutane, Retin-A, strong retinoids, etc.) or have taken prescribed skin medications within the last year. MUST BE OFF OF SKIN MEDICATIONS FOR 1 YEAR IN ORDER TO GET PERMANENT MAKEUP DONE.
    - You are epileptic or who has experienced faint spells or seizures.
    - You have had previous eyebrow tattoos done (previous ombré powder or microblading) that has NOT been approved by Alexandra.

    Please notify us if you have/had allergic reactions to topical anesthetics.
    Please notify us of any other medications you are currently taking.
    Please notify us if you have a health problem or disease not listed above.

    If you have other health complications or diseases not listed above, please consult your doctor to see if permanent makeup is a safe procedure for you. If approved by your doctor, please notify Alexandra of your condition via call or text at (619) 786-3232. A legitimate doctor's note from a visit that is scheduled no more than one week before your appointment is required to proceed with your appointment. We do not want to put your health in jeopardy, as well as the artist's health.

    If you have already booked an appointment and have to cancel due to the health contraindications, your deposit will be forfeited. If you have a temporary health contraindication (i.e. underage, pregnancy, taking Accutane, etc.), you will also have to forfeit your deposit and submit a new deposit for a future booking once you are cleared of all health contraindications. If you decide to show up to your appointment with any of these disqualifications, the service will be refused and you will be automatically charged 100% of the appointment price.

     Health Contraindications for Brow Lamination:

    I am informing my esthetician of any of the following contraindications. I understand if any of these apply to me, it is NOT advised to have the procedure.

    -Allergies to adhesives and fumes
    -Currently going through Chemotherapy
    -Psoriasis
    -Eczema
    -Have used retinol, accutane, BHA/AHA or exfoliated near the eyebrow area in the last week. (Includes peels)
    -Alopecia
    -Sunburn (on face)
    -Ultra sensitive skin
    -Wound on treated area
    -Recent eye surgery
    -Recent Permanent makeup
    -Pregnant or nursing
    -Taken blood thinners
    -Botox in the last 2-3 weeks
    -Spray tan in the last 48 hours

    It is always possible to have a reaction in regards to any new procedure we undertake. Please review and check the boxes as you go along to make it clear you are aware and have taken all of this into consideration.

  • By signing up below, I verify that I have read, understand and agree with the above statements. I acknowledge my consent as initialed above, and I further recognize this consent form will supersede any other photo consent forms with a date prior to the date written below. This consent may be revoked at any time by request or by completion of a new form. Further, I release and discharge KALINAW.STUDIOS of any and all claims or actions that I have relating to such use and publication.
