1. The resident will submit to random drug testing and random inspection of living areas and phone.
2. Other than resident’s cell phone, NO electronics including computers, tablets, notebooks, or game systems of any kind are permitted on campus or grounds.
3. The resident will abide by curfew rules.
4. The Resident understands that the entire facility and grounds are tobacco-free.
5. The resident will have employment with Christian leadership.
6. The resident cannot work Sundays.
7. The resident must attend all Midway services.
8. The resident will be a member of Midway Baptist Church.
9. The resident will participate in Christ-centered Bible studies with Pastors and discipleship groups.
10. The resident will complete basic life skills courses which may include financial training, GED programs, parenting classes, etc.
11. The resident will assist with the maintenance and general upkeep of the building and grounds of Midway Baptist Church.
12. The resident will participate in mission outreach projects and all evangelism opportunites and activities.
13. Residents will be expected to keep all living areas neat and tidy.
14. Residents will have access to kitchen facilities, but will provide their own food and are responsible for any necessary cleanup after use.
15. Residents are allowed to have visitors attend services at Midway Baptist, but no visitors are allowed on the property or grounds at any other time.
16. Residents will be granted passes at the discretion of the Director. (unless otherwise, court ordered or as directed by Community Corrections and/or probation or parole officer).
17. Visitation Policy: 1. Director must approve all visits. 2. All visitors must attend worship service prior to visit. 3. All visits are to be held ONLY in the following areas: Fellowship Hall at Anchor 1, Dining/Kitchen area, or under the pavilion. 4. Visitors are NOT allowed in ANY Anchor Residential area at anytime under any circumstance.

By my signature, I state that I have completed this application truthfully to the best of my ability. I further acknowledge that I have read, understand and agree to abide by all rules and responsibilities set forth by Anchor Ministry a ministry of Midway Baptist Church. Additionally, I understand failure to do so, may result in immediate program termination & dismissal from all Midway Baptist Church grounds and property.
