Permission to Use Image/Testimonial Data
I,   First Name   Last Name   give LEAD Brevard, the absolute right and unrestricted permission to take, use my name, testimonial and biographical data and/or publish, reproduce, edit, exhibit, project, display and/or copyright photographic images, pictures, video or recordings of me through any form of media (print, digital, electronic, broadcast or otherwise) for art, advertising, recruitment, marketing, fundraising, publicity, archival or any other lawful purpose.
I waive any right that I may have to inspect and approve the finished product that may be used or to which it may be applied now and/or in the future, whether that use is known to me.
I understand that virtual programming may be recorded and grant permission for LEAD Brevard and/or any person(s) or entities authorized on LEAD Brevard’s behalf to record, tape, film, photograph, edit, alter, digitize, or otherwise capture and preserve in permanent from my name, likeness, biographical material, voice, and/or performance/display. I further grant permission for LEAD Brevard or any person or entity authorized by it, to broadcast, publish, or transmit all or any portion of the recording or altered/edited recording in any form, via any means.

ENROLLMENT AGREEMENT ATTENDANCE REQUIREMENT:

If emergency/circumstances arise that prevent a participant from attending all or any portion of ONE SESSION, the participant may be excused from attending all or a portion of one session. If for any reason a class member misses TWO SESSIONS, he/she will be removed from the program and will not be eligible to graduate that year.

 STATEMENT OF COMMITMENT:

I   First Name   Last Name  understand the commitment required to participate in Leadership Essentials and will devote the time necessary to complete the program. This includes attendance at each of the six half-day program sessions and the Graduation Ceremony. If I am unable to comply with the requirements, I will voluntarily withdraw from the program. I understand that, in the interest of the class experience for all participants, any participant may be removed from the class if, in the opinion of LEAD Brevard, that participant’s conduct is not conducive to the work of the group or there is a failure to meet participation requirements. I will be respectful of the program facilitators/presenters and my peers if/when accessing my mobile device for program related purposes. I further understand that tuition is NOT refundable in whole or in part after October 3, 2023.
