I hereby grant Lisa Patterson Raterman and Working Mindfulness LLC the right to use in whole or part of any recorded footage in which my name and voice may be included (hereinafter “Recordings”) whether recorded on or transferred to video, audio, digital format, or other media not known or developed. The Recordings will ONLY be used for non-commercial, educational purposes to further the MBSR certification process for the instructor and will only be used for the educational review by the instructor’s mentor at Brown University Mindfulness Center. The Recordings will be destroyed within 6 months of the end of class.
I hereby waive any and all right that I may have to inspect or approve the Recordings that may be used for educational purposes.
I am over the age of 18 and have read the release, and fully understand the contents.
